摘要
目的探讨急性白血病患儿血清转铁蛋白受体(TFR)的表达水平及其临床意义。方法选取82例急性白血病患儿为观察组,选取50例健康儿童为对照组;比较两组受试者血清中铁蛋白(SF)、转铁蛋白(TF)、TFR的水平及TFR mRNA水平,并对TFR mRNA水平与急性白血病患儿临床特征、疗效和预后的关系进行分析。结果观察组受试者的血清SF、TF、TFR蛋白水平及TFR mRNA水平均明显高于对照组(P﹤0.01)。不同白细胞(WBC)计数、白血病细胞计数、白血病危险分级急性白血病患儿的TFR mRNA相对表达量比较,差异均有统计学意义(P﹤0.01)。治疗后,急性白血病患儿的TFR mRNA相对表达量明显低于治疗前(P﹤0.01);完全缓解组患儿的TFR mRNA相对表达量低于部分缓解组和未缓解组,部分缓解组患儿的TFR mRNA相对表达量低于未缓解组,差异均有统计学意义(P﹤0.05)。Cox比例风险回归模型分析结果显示,TFR mRNA为急性白血病患儿预后的危险因素(RR=1.276,95%CI:1.105~1.467,χ^2=13.562,P﹤0.05)。结论急性白血病患儿的TFR mRNA表达水平明显增高,且其表达水平与患儿的疾病程度、临床疗效和预后密切相关。
Objective To investigate the expression of serum transferrin receptor(TFR)in children with acute leukemia and its clinical significance.Method 82 children with acute leukemia were selected as observation group and 50 healthy children as control group.The levels of serum ferritin(SF),transferrin(TF),TFR and TFR mRNA were compared between the two groups,and the relationship between TFR mRNA and clinical characteristics,efficacy and prognosis of children with acute leukemia was analyzed.Result The levels of serum SF,TF,TFR protein and TFR mRNA in observation group were significantly higher than those in control group(P<0.01).There were significant differences in the relative expression of TFR mRNA in children with acute leukemia with different white blood cell(WBC)counts,leukemia cell counts and leukemia risk grades(P<0.01).The relative expression of TFR mRNA in acute leukemia children after treatment was significantly lower than that before treatment(P<0.01);the relative expression of TFR mRNA in complete remission group was lower than that of children in partial remission group and non-remission group,and the relative expression of children in partial remission group was lower than that of children in non-remission group,both the difference was statistically significant(P<0.05).Cox proportional risk regression model analysis showed that TFR mRNA was a risk factor for prognosis of children with acute leukemia(RR=1.276,95%CI:1.105-1.467,χ^2=13.562,P<0.05).Conclusion The level of TFR mRNA from children with acute leukemia is significantly increased,and its expression level is closely related to the degree activity,clinical efficacy and prognosis of children with acute leukemia.
