摘要
目的:探讨24h蛋白尿定量在不同程度妊娠期高血压疾病(HDP)中的临床价值。方法:收集2016年10月1日至2018年12月31日在上海市嘉定区妇幼保健院产检并分娩的HDP孕产妇150例,分为妊娠期高血压组、子痫前期组、重度子痫前期组,每组50例,另设正常对照组50例。分析比较4组孕产妇的24h蛋白尿定量及母婴预后指标,24h蛋白尿定量与母婴预后指标的相关性。结果:4组孕产妇的血压、24h蛋白尿定量、分娩孕周、出血量、新生儿出生体重比较,差异均有统计学意义(P=0. 000)。4组孕产妇的24h蛋白尿定量与分娩孕周、新生儿出生体重、新生儿Apgar评分均显著相关(P=0. 000)。以24h蛋白尿定量≥0. 3g/L定为子痫前期,以24h蛋白尿定量≥2g/L定为重度子痫前期,发现24h蛋白尿定量与HDP的严重程度不呈正相关。结论:24h蛋白尿定量在不同程度HDP中的作用不一样,其与HDP的严重程度不呈正相关。
Objective: To assess the clinical effects of 24-hour urinary protein quantification in different degrees of hypertensive disorders in pregnancy. Methods: 150 women who were examined and delivered in Shanghai Jiading Maternal and Child Care Service Center from Oct. 1,2016 to Dec. 31,2018 were divided into three groups: hypertensive pregnancy group,preeclampsia group,severe preeclampsia group,50 cases in each group. And another 50 women in normal control group. To analyze the difference of 24-hour urinary protein quantification and prognostic indicators in four groups,and the correlation between 24-hour urinary protein quantification and prognostic indicators. Results: There were significant differences in blood pressure,24-hour urinary protein quantification,gestational weeks of delivery,blood loss and neonatal weight among the four groups( P = 0. 000). There were significant correlations between 24-hour urinary protein quantification and gestational weeks of delivery,neonatal weight and neonatal Apgar score among the four groups. There was no positive correlation between 24-hour urinary protein quantification and severity of pregnancy-induced hypertension if preeclampsia was diagnosised as 24-hour urinary protein quantification( ≥0. 3 g/L) and severe preeclampsia was diagnosised as 24-hour urinary protein quantification( ≥ 2 g/L). Conclusion: 24-hour urinary protein quantification plays different roles in different degrees of pregnancy-induced hypertension,there was no positive correlation between 24-hour urinary protein quantification and severity of pregnancy-induced hypertension.
