吞咽康复、电刺激联合冰刺激治疗脑卒中吞咽障碍探讨 被引量:7

摘要 目的探讨临床收治的脑卒中患者加强行吞咽康复训练、低频电刺激治疗及冰刺激治疗对吞咽功能和预后的改善作用。方法选择脑卒中患者80例,均为我院神经内科2015年1月~2017年1月收治,随机分组,单纯采用吞咽康复训练方案治疗(对照组,n=40)与加行电刺激、冰刺激方案治疗(观察组,n=40)对吞咽功能、总有效率、呼吸功能、神经功能缺损造成的影响展开比较。结果两组脑卒中患者治疗前吞咽功能评分比较,差异无统计学意义(P>0.05),治疗后均有程度不等的降低,但相较对照组,观察组降幅更为显著,差异有统计学意义(P<0.05)。观察组患者总有效率经评定为95%,明显高于对照组77.5%,差异有统计学意义(P<0.05)。两组治疗前呼吸功能指标PEF、FEV1、FVC比较,差异无统计学意义(P>0.05),治疗后均有程度不等改善,但相较对照组,观察组改善幅度更为显著,差异有统计学意义(P<0.05)。两组治疗前神经功能NIHSS评分比较差异无统计学意义(P>0.05),治疗后均有程度不等下降,但相较对照组,观察组降幅更为显著,差异有统计学意义(P<0.05)。结论针对临床收治的脑卒中患者,在行吞咽康复基础上,联合行电刺激、冰刺激治疗,可显著增强吞咽功能,呼吸功能,改善总有效率,促进神经功能恢复,具非常重要开展价值。 Objective To explore the effect of swallowing rehabilitation,electrical stimulation combined with ice stimulation for patients with stroke who were clinically admitted on the improvement of dysphagia and prognosis. Methods 80 patients with stroke who were admitted and treated in department of neurology in our hospital from January 2015 to January 2017 were selected and they were randomly divided into two groups.40 patients in the control group were treated with swallowing rehabilitation exercise regimen while 40 patients in the observation group were treated with regimen of swallowing rehabilitation exercise,electrical stimulation combined with ice stimulation.Swallowing function,total effective rate,respiratory function and the effect of neurologic impairment were compared. Results There was no statistically significant difference in the swallowing function scores before treatment between the two groups(P>0.05).Swallowing function scores after treatment had varying degrees of reduction.Compared with the control group,the decrease was more significant in the observation group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was 95%,which was significantly higher than that of the control group(77.5%).The difference was statistically significant(P<0.05).There was no statistically significant difference in respiratory function indexes such as PEF,FEV1 and FVC between the two groups before treatment(P>0.05).After treatment,there were varying degrees of improvement.Compared with the control group,the improvement was more significant in the observation group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in NIHSS scores of neurological function between the two groups before treatment(P>0.05).After treatment, there were varying degrees of improvement.Compared with the control group,the reduction was more significant in the observation group,and the difference was statistically significant (P<0.05).
