摘要
目的探讨半剂量维替泊芬光动力疗法(photodynamic therapy,PDT)治疗中心性浆液性脉络膜视网膜病变(central serous chorioretinopathy,CSC)的临床疗效和安全性。方法病例回顾性研究。选择15例(17只眼)确诊CSC患者,患眼病程3〜120个月。予以半剂量PDT治疗,治疗后随访6〜12个月,分析比较患者PDT前后的最佳矫正视力(BCVA)、间接眼底镜检查、荧光素眼底血管造影(FFA)和光学相干断层扫描(OCT)结果。对同一患眼治疗前后BCVA和黄斑中心凹区视网膜厚度(central foveal thickness,CFT)比较,采用配对f检验。结果患眼经半剂量PDT治疗后末次随访时BCVA为(0.89±0.42),与治疗前BCVA比较,差异有统计学意义(t=3.58;P<0.05)。治疗后末次随访时OCT检查示CFT为(204.00±45.13)μm,与治疗前(413.18±118.69μm)比较,差异有统计学意义(t=-6.289,P<0.05)。患者病程时长与PDT治疗前后BCVA提高值显著相关(r=-0.494,P<0.05)。随访期间患眼视力稳定,病变无复发。结论半剂量维替泊芬光动力疗法对CSC患者的治疗安全有效。为了不导致患者视力不可逆的损伤,还应尽早积极地治疗。
Objective To investigate the clinical efficacy and safety of a half dose of Verteporfin photodynamic therapy in the treatment of central serous chorioretinopathy.Methods This is a case of retrospective study,15 CSC patients(17 eyes)were treated with a half dose of Verteporfin PDT therapy.The courses of the disease varied during 3 to 120 months.The patients were followed 6 to 12 months after treatment.BCVA,FFA and OCT examinations were checked and compared before and after the PDT treatment.The BCVA,central fovea thickness(CFT)of the same patient pre and post-PDT were analyzed by Paired t-test.Results The BCVA of the patient treated with half dose PDT was 0.89±0.42 as their last follow-up,which was significantly improved than those of pre-PDT(t=3.58,P<0.05).The CFT after treatment at their last visit was 204.00±45.13um,which was significantly thinner than those before treatment(413.18±118.69um,t=-6.289,P<0.05).The duration of the disease courses was significantly associated with the improvement of BCVA before and after PDT treatment(r=-0.494,P<0.05).During the follow-up period,the PDT treated eyes were stable and the lesion had no recurrence.Conclusions The treatment of half dose verteporfin photodynamic therapy for CSC patients is safe and effective.In order not to avoid irreversible vision damage,the CSC patients should be treated as early as possible.
