摘要
目的比较撕囊碎核镊辅助白内障预劈核技术和拦截劈核技术,探讨撕囊碎核镊辅助预劈核的技术优点。方法将年龄相关性白内障46例54只眼随机分为撕囊碎核镊辅助预劈核组(A组)23例25只眼、拦截劈核组(B组)23例29只眼。分别使用撕囊碎核镊辅助预劈核、拦截劈核技术进行白内障超声乳化摘除术。记录术中实际超声乳化时间(U/S time)、手术时间、术前术后角膜内皮细胞计数、角膜内皮细胞丢失率、术中并发症、术后1d裸眼视力和术后1d角膜水肿等情况并进行比较。结果将两组进行比较,A组的U/S time,手术时间均低于B组;术后第ld角膜水肿A组轻于B组。术后1d两组平均裸眼视力无统计学差异。A组术后3个月平均角膜内皮细胞计数高于B组,A组术后3个月平均角膜内皮细胞丢失率低于B组。术中A组出现晶状体后囊膜破裂有1只眼,B组有2只眼。结论与拦截劈核技术相比,撕囊碎核镊辅助预劈核技术操作简单,超声时间、手术时间更短,术后3个月角膜内皮细胞丢失率更低,手术安全且并发症少。
Objective To discuss the advantages of capsulorhexis-chop forceps assisted prechop(CCFP)technique compared with stop and conquer.Methods 46 patients(54 eyes)with age related cataract were randomly divided into 2 groups,CCFP were performed in group A including 23 patients(25 eyes),stop and conquer were performed in group B including 23 patients(29 eyes).The U/S time,operation time,preoperative and postoperative corneal endothelial cell density,corneal endothelial cell loss rate,intraoperative complications,postoperative(UCVA)at 1d and corneal edema were reported.Results The U/S time and operation time were shorter in group A than group B.On the first day after surgery,corneal edema was less severe in group A than group B.There is no significant difference between two groups in UCVA on the first day after surgery.The average corneal endothelial cell density was higher in group A than group B,The average corneal endothelial cell loss rate was lower in group A than group B after 3 months.One eye in group A had posterior capsule rupture,2 eyes in group B.Conclusions CCFP technique is easy and safe to operate with less complications,the time of ultrasound and the operation time are shorter,the loss rate of cormeal endothelial cells is lower compared with stop and conquer.
