摘要
目的:研究益气除痞汤对反流性食管炎患者的治疗效果及血清炎性因子表达的影响。方法:选择脾虚郁滞型反流性食管炎的患者100例,随机分为对照组和治疗组各50例,对照组给予奥美拉唑肠溶片联合莫沙必利片治疗,治疗组中药汤剂益气除痞汤口服,分别干预8周后观察其对反流性食管炎临床症状、胃镜下表现和血清炎性因子相关指标的影响。结果:主要中医症状综合疗效评定,治疗组的总有效率为88.00%,显著高于对照组的78.00%(P<0.05);在改善胃镜下病变方面,治疗组的总有效率为86.00%,显著高于对照组的82.00%(P<0.05);与对照组相比,治疗组能进一步降低反流性食管炎血清炎性因子白细胞介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)表达(P<0.05)。结论:益气除痞汤对脾虚郁滞型反流性食管炎患者临床疗效确切,可缓解反流性食管炎临床症状,改善中医症候积分,其改善临床疗效的作用机制可能与降低血清炎性因子IL-1、TNF-α水平相关。
Objective:To investigate the curative effect of Yiqi Chupi Decoction on reflux esophagitis and the influence on serum inflammatory factors in esophageal mucosa. Methods:One hundred patients with reflux esophagitis on spleen deficiency and stagnation were selected as the subjects,who were randomly divided into the control group and the observation group,with 50 cases in each. The control group was treated with omeprazole enteric-coated tablets and osapride tablets while the observation group was treated with Yiqi Chupi Decoction for eight weeks. The clinical efficacy and endoscopic manifestations was compared between the two groups after treatment. Serum inflammatory factors[interleukin-1(IL-1),tumor necrosis factor α(TNF-α)〕of the two groups were detected before and after treatment. Results:The total effective rate of the observation group was higher than that of the control group(88.00% vs 78.00%)(P<0.05).The endoscopic manifestations effective rate of the observation group was higher than that of the control group(86.00% vs 82.00%)(P<0.05). After treatment,optical densities of expressions of IL-1 and TNF-α were significantly decreased in both groups,especially in the observation group(P<0.05). Conclusion:Yiqi Chupi Decoction can effectively improve the curative effect and the integral of TCM symptoms of reflux esophagitis on spleen deficiency and stagnation. The mechanism may be related to significant reduction of serum inflammatory factors.
