急性脑梗死患者网膜素-1水平的动态变化的临床研究 被引量:2

摘要 目的 探讨网膜素-1(omentin-1)与急性脑梗死的相关性,明确网膜素-1在急性脑梗死预测和病情演变方面的临床意义.方法 选取2017年7月-2019年2月在建昌县人民医院神经内科住院的95 例急性脑梗死患者为脑梗死组.92 名同期进行健康体检者为对照组.记录受试者的基本信息;测量身高和体重,计算BMI;检测空腹血糖、血压、血脂等指标,并通过颅脑CT和磁共振评估脑梗死面积和部位.入院时进行NIHSS评分及GCS评分.分别在入院2 d,7 d和14 d清晨空腹抽取脑梗死组肘静脉血,ELISA法检测网膜素-1.将脑梗死患者根据是否合并糖尿病,是否合并高血压,牛津郡社区卒中分型(OCSP)及预后情况分别区分亚组,对网膜素-1水平进行分析评价.分析网膜素-1与神经功能损伤NIHSS评分、昏迷评分GCS以及预后情况mRS的相关性,分析影响网膜素-1和预后mRS评分的危险因素.结果 急性脑梗死患者入院后2、7、14 d,脑梗死组血清网膜素-1水平显著低于对照组(P<0.01),2 d、7 d以及14 d脑梗死组血清网膜素-1水平差异无统计学意义(P>0.05).脑梗死合并糖尿病亚组和脑梗死合并高血压亚组网膜素-1水平显著降低(P<0.05).完全前循环梗死(TACI)组、部分前循环梗死(PACI)组及后循环梗死(POCI)组患者血清网膜素-1水平显著低于腔隙性脑梗塞(LACT)组(P<0.05).预后较好组患者血清网膜素-1水平显著升高(P<0.05).网膜素-1与GCS评分正相关,与NIHSS评分和mRS评分负相关.空腹血糖、TG以及梗死部位是脑梗死患者血清网膜素-1的影响因素,入院14 d网膜素-1是脑梗死患者预后的影响因素.结论 网膜素-1为脑梗死发生和预后的有效预测指标之一.网膜素-1是脑梗死重要的保护因子,提高网膜素-1水平,可降低患者神经功能损伤,改善脑梗死患者预后. Objective To explore the correlation between omentin-1 and acute cerebral infarction,and to clarify the clinical significance of omentin-1 in the prediction and treatment of acute cerebral infarction.Methods 95 patients with acute cerebral infarction who were admitted to the department of neurology of Jianchang People's Hospital from July 2017 to Feb 2019 were enrolled in the cerebral infarction group.92 healthy subjects were selected as the control group.Basic informations about the subjects were recorded.Height and weight were measured and BMI was calculated.Fasting blood glucose,blood pressure,blood lipids,cerebral infarction area and position were measured.NIHSS scores and GCS scores were performed at admission.Blood specimens were taken for studying from veins in the morning on the 2nd,7th,14th day after admission respectively,and omentin-1 was detected by ELISA.Patients with cerebral infarction were divided into subgroups according to diabetes mellitus,hypertension,Oxfordshire Community Stroke Project(OCSP)classification and prognosis,and omentin-1 level was evaluated.The correlation between omentin-1 and NIHSS,GCS and mRS and the risk factors affecting omentin-1 and mRS were analyzed.Results The levels of serum omentin-1 in patients with acute cerebral infarction were significantly lower than those in the control group on the 2nd,7th and 14th day after admission(P<0.01).There was no significant change in serum omentin-1 levels between the 2nd,7th and 14th day of cerebral infarction group(P>0.05).The levels of omentin-1 in cerebral infarction combined with diabetes and cerebral infarction combined with hypertension was significantly reduced(P<0.05).Compared with the LACT group,serum omentin-1 levels were significantly lower in the TACI group,PACI group,and POCI group(P<0.05).Serum omentin-1 levels were significantly higher in the group with better prognosis(P<0.05).Omentin-1 was positively correlated with GCS and negatively correlated with NIHSS and mRS.Fasting blood glucose,TG and cerebral infarction posit
