基于X线全自动冠状动脉造影血流储备分数在冠心病介入诊治中的应用探讨 被引量:4

摘要 目的初步探究基于X线全自动冠状动脉造影血流储备分数(CAFFR)和传统有创血流储备分数(FFR)相比在冠心病介入诊治方面的应用价值。方法纳入接受冠状动脉造影(CAG)及FFR检查的可疑或确诊冠心病的患者38例,平均年龄(60.47±8.2)岁。采用基于X线冠状动脉造影图像的CAFFR计算方法进行FFR值测定,以有创性FFR结果作为金标准,分析比较CAFFR在冠心病患者介入诊治中的准确度。采用Pearson相关性检验及Bland-Altman方法比较CAFFR与FFR的诊断相关性及一致性。结果以病变血管为分析单位FTR>0.8为26支,CAFFR>0.8为25支,FFR≤0.8为16支,CAFFR≤0.8为7支,CAFFR与FFR诊断一致性及相关性良好(r=0.661,P<0.01)。结论CAFFR在冠心病患者介入诊治中具有良好的临床应用价值。 Objective To explore the application value of coronary artery angiography based on automatic X-ray coronary angiography fractional flow reserve(CAFFR)compared with traditional invasive flow reserve(FFR)in the diagnosis and treatment of coronary artery disease.Methods A total of 38 patients with suspected or confirmed coronary heart disease were included in the study,with an average age of(60.47±8.2)years.CAFFR calculation method based on X-ray coronary angiography image was used to measure the FFR value,and the invasive FFR results were used as the gold standard to analyze and compare the accuracy of CAFFR in the interventional diagnosis and treatment of coronary heart disease.Pearson correlation test and Bland Altman method were used to compare the diagnostic correlation and consistency between CAFFR and FFR.Results There were 26 vessels with FFR>0.8,25 vessels with CAFFR>0.8,16 vessels with FFR≤O.8 and 7 vessels with CAFFR≤0.8.The diagnosis of CAFFR and FFR was consistent and correlated well(r=0.661,P<0.01).Conclusions CAFFR has good clinical value in interventional diagnosis and treatment of coronary artery disease.
