冻融胚胎移植中子宫内膜厚度及移植时间对异位妊娠发生率的影响 被引量:4

摘要 目的探讨子宫内膜厚度及移植时间对冻融胚胎移植(FET)异位妊娠(EP)发生率的影响。方法回顾性分析西北妇女儿童医院生殖中心行FET的11861个周期的妊娠结局,其中6901周期临床妊娠,75周期异位妊娠。根据移植时间分为D 3移植组(胚胎5912周期),D 5移植组(囊胚5949周期);每组再根据子宫内膜厚度,将患者分为≤9 mm组、>9~12 mm组和>12 mm组,比较各组间异位妊娠的发生情况。结果D 3移植组发生异位妊娠45例,D 5移植组发生异位妊娠30例,2组异位妊娠发生率比较差异有统计学意义(P<0.05)。与≤9 mm组比较,>9~12 mm组和>12 mm组的异位妊娠发生率均显著较低(P<0.05)。结论冻融胚胎移植时D 5移植的异位妊娠风险明显较D 3胚胎移植低,子宫内膜厚度可作为ART异位妊娠的独立危险因素。 Objective To investigate the effect of the endometrial thickness and embryo transplantation(ET)time on the incidence of ectopic pregnancy in frozen thawed embryo transplantation.Methods A prospective study of 11681 frozen-thawed embryo transplantation cycles in the Reproductive Center of Northwest Women and Children′s Hospital was conducted to analyze the pregnancy outcomes,including 6901 cycles of clinical pregnancy and 75 of ectopic pregnancy.According to the embryo transplantation time,the FET cycles were divided into D 3 embryo transplantation group(5912 cycles in embryo period)and D 5 blastula transplantation group(5949 cycles in blastula period).And were further subdivided into≤9 mm group,>9~12 mm group and>12 mm group according to endometrial thickness.The incidence of ectopic pregnancy of varied groups were compared.Results There were 45 cases with ectopic pregnancy in the D 3 embryo transplantation group,and 30 cases in D 5 blastocyst transplantation group,and its incidence of ectopic pregnancy showed a significant difference(P<0.05).Compared with≤9 mm group,the incidence of ectopic pregnancy in>9~12 mm group and>12 mm group were significantly lower.Conclusion D 5 blastula transplantation group has significantly lower risk of ectopic pregnancy than D 3 embryo transplantation group,endometrial thickness can be independent factor of ectopic pregnancy.
