摘要
目的探讨髋臼内陷技术行生物型全髋关节置换术(THA)治疗髋关节发育不良(DDH)的疗效。方法采用髋臼内陷技术行THA治疗39例DDH患者(39髋)。记录双下肢长度差异、髋关节中心内移距离、臼杯内突率、髋关节中心高度、臼杯覆盖率、Harris髋关节评分(HHS)、臼杯松动、骨长入等。结果患者均获得随访,时间2~5(3.4±1.5)年。末次随访时HHS由术前32~55(43.3±15.5)分提高到82~98(91.2±11.8)分;双下肢长度差异由术前1~5(3.1±1.1)cm下降到术后0~2(0.6±0.3)cm。术后髋关节中心高度2~3(2.4±0.8)cm,髋关节中心内移1~4(2.5±0.9)cm,臼杯内突率21%~64%(42.3%±12.5%),臼杯覆盖率81%~100%(96.7%±5.2%)。至末次随访,假体均获得骨长入性固定,未发生臼杯松动。结论采用髋臼内陷技术行THA治疗DDH,疗效满意。
Objective To investigate the efficacy of the socket medialization technique in the treatment of developmental dysplasia of the hip(DDH)with total hip arthroplasty(THA).Methods The 39 patients(39 hips)with DDH were treated with the socket medialization technique.The difference in length of the lower limbs,the distance of hip center medialization,the medial protrusion rate of prosthesis acetabulate cup,the coverage rate of acetabular cup,the height of hip center,the Harris hip score(HHS),the loosening of the acetabular cup,and the ingrowth of the bone were recorded.Results All patients were followed up for 2~5(3.4±1.5)years.At the last follow-up,HHS increased from 32~55(43.3±15.5)points to 82~98(91.2±11.8)points.The difference in length of both lower limbs decreased from 1~5(3.1±1.1)cm before surgery to 0~2(0.6±0.3)cm after surgery;the postoperative height of hip center was 2~3(2.4±0.8)cm;the postoperative distance of hip center medialization was 1~4(2.5±0.9)cm;the postoperative medial protrusion rate of acetabulate cup was 21%~64%(42.3%±12.5%);the rate of cup coverage was 81%~100%(96.7%±5.2%).At the last follow-up,the prosthesis was obtained with bone ingrowth fixation,and no acetabular cup was loosened.Conclusions The socket medialization technique in THA for DDH,can achieve a satisfactory result.
