摘要
目的研究自拟复元骨愈汤在诱导股骨假体周围骨生长中的效果。方法选择2016年6月~2017年6月于我院行全髋关节置换术的患者60例,将患者按就诊顺序采用随机数字法分为两组,治疗组、对照组各30例。治疗组术后第3天服用自拟复元骨愈汤2个疗程,共2周;对照组采用常规治疗。观察术后3个月、6个月、1年股骨假体周围骨长入"点焊"个数、股骨假体远端CT值、Harris评分等指标。结果治疗组术后3个月、6个月、1年假体周围点焊个数明显多于对照组,差异有统计学意义(P<0.05);治疗组术后3个月假体远端CT值与对照组比较,差异无统计学意义(P>0.05),术后6个月、1年假体远端CT值高于对照组,差异有统计学意义(P<0.05)。各组术后Harris评分比较,无统计学差异(P>0.05)。结论自拟复元骨愈汤对股骨假体周围骨生长具有诱导作用,能刺激和加快骨长入假体,对指导人工关节置换、特别是对骨质疏松患者术中技术改进、术后用药、加快康复等预后具有临床指导意义。
Objective To study the role of self-made Fuyuan Guyu Decoction in inducing the bone growth around femoral prosthesis through comparative study.Methods The 60 patients who were treated with total hip replacement surgery in our hospital from June 2016 to June 2017 were randomly divided into the treatment group and the control group according to the order of treatment,with 30 patients in each group.On the third day after surgery,the treatment group was given oral administration of self-made Fuyuan Guyu decoction for 2 treatment courses and each course was2 weeks,and the control group was treated with conventional treatment.The number of bones around femoral prosthesis growing into the"spot welding",CT value of distal femoral prosthesis and Harris scores 3 months,6 months and 1 year after surgery were observed.Results The numbers of spot welding around the prosthesis in the treatment group were significantly higher than those in the control group 3 months,6 months and 1 year after surgery,with statistically significant differences(P<0.05).The CT value of distal prosthesis 3 months after surgery was not statically different from that of the control group(P>0.05),but the CT values of the distal prosthesis were higher than those of the control group 6 months and 1 year after surgery,with statistically significant differences(P<0.05).There was no significant difference in the Harris score between the two groups(P>0.05).Conclusion Self-made Fuyuan Guyu decoction can induce the bone growth around femoral prosthesis,stimulate and accelerate the growth of bone into the prosthesis.It has clinical significance for the guidance of prosthetic replacement,especially for technical improvement,postoperative medication and rehabilitation acceleration in patients with osteoporosis.
