摘要
目的探讨清肺平喘汤治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床疗效。方法选取我院2017年10月~2018年9月收治的慢性阻塞性肺疾病急性加重期中医辨证属痰热郁肺证患者80例为研究对象,按数字随机法分为对照组和观察组,每组40例。对照组给予常规西药治疗,治疗组在对照组基础上加用清肺平喘汤。比较两组的临床疗效、中医证候评分、肺功能、血气分析、IL-8、TNF-α、PCT和CRP的水平变化。结果治疗后,观察组总有效率明显高于对照组(P<0.05);观察组咳嗽、咯痰、气短、喘息、湿啰音积分均低于对照组,FEV1、FVC、FEV1%水平高于对照组(均P<0.05)。治疗后,观察组PaO2、SaO2水平高于对照组,PaCO2水平低于对照组(均P<0.05);观察组IL-8、TNF-α、CRP及PCT明显低于对照组(P<0.05)。结论清肺平喘汤联合西药治疗慢性阻塞性肺疾病急性加重期效果显著。
Objective To observe the clinical effect of Zhike Pingchuan decoction in the treatment of acute exacerbation chronic obstructive pulmonary disease.Methods 80 patients with acute exacerbation chronic obstructive pulmonary disease from October 2017 to September 2018 were randomly divided into treatment group and control group.The control group was treated with conventional western medicine,while the treatment group was treated with Zhike Pingchuan decoction on the basis of the control group.The clinical efficacy,TCM score,pulmonary function,blood index,IL-8,TNF-α,CRP and PCT levels were observed before and after treatment.Results The total effective rate in treatment group was higher than that in control group(P<0.05).After treatment,the score of cough,sputum expectoration,shortness of breath,wheezing and moist rale in the treatment group were significantly lower than those in control group(P<0.05).The levels of FEV1,FVC and FEV1%in the treatment group were significantly higher than those in control group(P<0.05).After treatment,the PaO2 and SaO2 levels in the in the treatment group were significantly higher than those in control group(P<0.05),and PaCO2 levels in the treatment group were significantly lower than those in control group(P<0.05).The IL-8,TNF-α,CRP and PCT in the treatment group were significantly lower than those in control group(P<0.05).Conclusion The effect of acute exacerbation chronic obstructive pulmonary disease treated by Zhike Pingchuan decoction combined with western medicine is remarkable.
