摘要
目的探讨荧光偏振检测技术(fluorescence polarization assay,FPA)在诊断人布鲁菌感染中的价值.方法选取115例布鲁菌病确诊患者,包括65例急性患者和50例慢性患者,同时选取81例布病疑似患者和90例健康体检者,采集静脉血,应用FPA和试管凝集试验(standard tube agglutinationtest,SAT)进行检测,以SAT检测结果为诊断标准,分析FPA在布鲁菌病诊断中的价值.结果确诊病例组、疑似病例组和健康对照组FPA检测人血清布鲁菌抗体阳性率分别为95.65%、4.94%和4.44%,三组间差异有统计学意义(χ^2=209.13,P<0.05).确诊病例组FPA检测人血清布鲁菌抗体阳性率显著高于疑似病例组和健康对照组,差异有统计学意义(χ^2值分别为137.21和146.66,P值均<o.05).急性布病组FPA测定值为(127.35±39.39)mp,明显高于慢性布病组(93.64±21.07)mp,差异有统计学意义(t=5.592,P<0.05).而急、慢性两组FPA检测布鲁菌抗体阳性率比较,差异无统计学意义(P>0.05).FPA检测对布鲁菌病的ROC曲线下面积(area under curve,AUC)为0.984,FPA在最佳诊断界值为80.5 mp时诊断敏感性为96.5%,特异性为99.3%.结论荧光偏振检测技术具有高敏感性和特异性等优点,能更简便快速地对布鲁菌病进行诊断.
Objective To investigate diagnostic value of fluorescence polarization assay(FPA)in human Brucella infection.Methods A total of 115 patients with confirmed Brucellosis were enrolled,including 65 acute cases and 50 chronic cases;and 81 suspected cases of brucellosis and 90 healthy subjects undergoing heahh check-up were served as controls.Venous blood was collected to detect Brucella antibody by FPA and standard tube agglutination test(SAT).Result of SAT test was taken as the golden criterial,and diagnostic value of FPA were analyzed.Result Positive rates of FPA tests in confirmed patients,suspected patients and healthy controls were 95.65%,4.94%and 4.44%,respectively.The differences among three groups had statistical significance(χ^2=209.13,P<0.05).Positive rates of FPA tests in confirmed patients were significantly higher than suspected patients and healthy controls(χ^2=137.21 and 146.66,all both P<0.05).The level of FPA for(127.35+39.39)mp in acute brucellosis group significantly higher than that for(93.64±21.07)mp in chronic brucellosis group(t=5.592,P<0.05).There was no significant difference between the two groups of acute and chronic of brucella antibody positive rates by FPA(P>0.05).The area under curve(AUC)of brucellosis detected by FPA was 0.984,and the sensitivity and specificity of FPA were 96.5%and 99.3%when the best diagnostic threshold was 80.5 mp.Conclusions FPA has the advantages of high sensitivity and specificity.It can be used for the diagnosis of brucellosis more simply and quickly.
