62例血透患者行中心静脉置管感染病原菌临床分析

Analysis of 62 hemodialysis patients with central venous catheter infections

摘要 目的了解血液透析患者行中心静脉置管感染病原菌分布及耐药情况,为临床治疗提供依据。方法选择2015年6月-2018年6月在本院行血透治疗感染62例患者,对其病原菌及耐药性进行分析。采用梅里埃公司VITEK 2 Compact全自动细菌分析鉴定仪进行细菌鉴定和药敏试验。结果共分离出病原菌96株,以革兰阳性菌为主,感染主要病原菌为金黄色葡萄球菌,对万古霉素、替考拉宁敏感,对大环内酯类、青霉素类等耐药率高。革兰阴性感染病原菌占主要数量的大肠埃希菌对头孢唑林、复方新诺明、环丙沙星等耐药率较高,对亚胺培南、头孢西林/他唑巴坦、头孢哌酮/舒巴坦耐药率相对较低。结论血透患者行中心静脉置管感染以革兰阳性菌为主,感染因素较多,应引起临床医生重视。 Objective To investigate the distribution and drug resistance of central venous catheter infection in hemodialysis patients, so as to provide basis for the clinical treatment. Methods A total of 62 hemodialysis patients with central venous catheter infections were enrolled in this study from June 2015 to June 2018, and then the pathogens and antimicrobial susceptibility of pathogens were analyzed. The identification and drug sensitivity test were conducted using VITEK 2-COMPACT auto bacteria analyzer. Results A total of 96 pathogenic isolates were obtained, Gram-positive bacilli was the major pathogen, where were mainly Staphylococcus aureus, and it was sensitive to vancomycin and teicoplanin, and had a higher resistance rates to macrolides, penicillins. Escherichia coli was the major Gram-negative bacill, which showed that Escherichia coli was high resistant to cefazolin, SMZ-TMP, ciprofloxacin, and the resistance rate of Escherichia coli to imipenem, Ceftriaxone/tazobactam and sulperazon were low. Conclusion Distribution of pathogen of central venous catheter infection in hemodialysis patients was based on Gram-positive bacteria. More infection factors should be noticed by clinicians.
