肾脏3D打印模型辅助复杂肾结石经皮肾镜取石术效果分析 被引量:8

摘要 目的探讨3D打印技术制作的复杂肾结石模型在经皮肾镜取石术(PCNL)中的应用效果.方法选取2015年1月至2017年12月在河北北方学院附属第一医院行PCNL的复杂肾结石患者14例(鹿角形结石9例、多发性肾结石5例).采用CT尿路造影采集数据,3D影像软件进行处理,光敏树脂制作3D复杂肾结石模型.在3D模型辅助下行PCNL.比较14例患者的3D打印模型与实际肾脏的长轴最长径、横轴最长径及结石最长径.结果14例复杂肾结石患者均成功构建肾脏3D模型,患者PCNL均顺利完成.术后第1天无残石率为85.7%(12/14),术后4周无残石率为100%(14/14),术后无并发症发生.14例患者肾脏长轴最长径、横轴最长径及结石最长径实际测量值与3D打印模型测量值比较,差异均无统计学意义(均P>0.05).结论3D打印肾脏模型真实还原了肾脏及结石周围的解剖结构,可协助PCNL顺利进行,对于降低手术并发症及提高碎石率有重要意义. Objective To observe the application of 3D printing model of percutaneous nephrolithotomy(PCNL)treating complex renal calculi.Methods From January 2015 to December 2017,14 patients with complex renal calculi(9 cases of staghorn calculi and 5 cases of multiple calculi)undergoing PCNL in the First Affiliated Hospital of Hebei North University were enrolled.Imaging data were collected by CT urography and analyzed by 3D image software,and 3D renal calculi model was printed using photosensitive resin.PCNL was performed with the reference of 3D printing model.The long diameters of kidney long axis,transverse axis and stones were measured.Results The 3D printing kidney model was constructed and PCNL was successfully performed in 14 patients.The stone-free rate was 85.7%(12/14)on the first day after operation,and it reached 100%(14/14)after 4 weeks.No complication was observed.There were no significant differences in the long diameters of kidney long axis,transverse axis and stones between the 3D printing model and actual measurements(all P>0.05).Conclusions The 3D printing technology can copy a model of kidney that shows anatomical details of organ structure and renal calculi.It provides reference for the operation of PCNL,which is helpful for reducing postoperative complications and improving the stone-removing rate.
