摘要
目的:探究对癫痫患者实施丙戊酸血药浓度监测在个体化给药方面的意义,为临床合理用药提供指导。方法:在2017年1月-2019年2月于笔者所在医院接受癫痫治疗的患者中随机选择184例为研究对象,并采用荧光偏振免疫法对入选研究对象的丙戊酸钠血药浓度实施监测,对不同血药浓度的临床治疗效果及用药期间的药物不良反应发生率实施对比分析。结果:未达到有效血药浓度(<50μg/ml)有45例,超过有效血药浓度(>100μg/ml)23例,处于有效血药浓度的患者为116例,处于有效血药浓度患者的治疗有效率较低于和高于有效血药浓度患者的治疗有效率高,不良反应随血药浓度的增加而有所增加,差异有统计学意义(P<0.05)。结论:合理的丙戊酸钠血药浓度可取得较好的疗效,但是个体差异较大,血药浓度的增加会导致不良反应发生率增加,应重视监测血药浓度开展个体化治疗。
Objective: To explore the significance of valproate concentration monitoring in individual prescription for epilepsy patients, and to provide guidance for clinical rational drug use. Method: From January 2017 to February 2019, 184 cases with epilepsy treated in our hospital randomly selected as the research object, and using the method of fluorescence polarization immunoassay for the research object of valproate blood drug concentration monitoring. The clinical therapeutic effect of different blood concentrations and the incidence of adverse drug reactions during drug implement were analyzed. Result: Not reach the effective blood drug concentration(<50 μg/ml) in 45 cases, more than the effective blood drug concentration(>100 μg/ml) in 23 cases, patients with the effective blood concentrations of 116 cases. The patients with the effective blood drug concentration efficient was higher than below and above the effective blood drug concentration, adverse reactions with the increase of the blood drug concentration increased, the difference was statistically significant(P<0.05). Conclusion: Reasonable serum drug concentration of valproate can achieve better efficacy, but the individual difference is large, the increase of blood drug concentration will lead to an increase in the incidence of adverse reactions, we should pay attention to monitor blood drug concentration to carry out individualized treatment.
