摘要
目的观察类风湿关节炎(RA)患者血清长链非编码RNA(LncRNA)-Hox转录反义RNA(HOTAIR)的表达变化,并探讨其临床意义。方法选择RA患者(RA组)132例,同期健康体检者50例(对照组)。取RA组入院第二天,对照组入院当天的空腹静脉血,采用实时RT-PCR法检测血清LncRNA-HOTAIR,ELISA法检测抗环瓜氨酸肽(CCP)抗体,速率散射比浊法检测RF、CRP,魏氏法检测ESR。分析RA患者血清LncRNA-HOTAIR水平与患者临床参数(DAS28,炎性指标及年龄、性别、病程、疼痛关节数)的关系。ROC分析血清LncRNA-HOTAIR水平对RA的诊断价值。结果RA组血清LncRNA-HOTAIR、抗CCP抗体、RF、CRP、ESR水平高于对照组(P均<0.05)。RA组病情高度活动者血清LncRNA-HOTAIR水平高于中度及低度活动者(P均<0.05)。RA组血清LncRNA-HOTAIR水平与DAS28呈正相关(r=0.645,P=0.009)。血清LncRNA-HOTAIR诊断RA的ROC下面积为0.706,敏感度71%,特异度82%。结论RA患者血清LncRNA-HOTAIR水平升高,其水平变化与疾病活动程度有关,对RA的诊断准确度较高。
Objective To observe the expression of long chain non-coding RNA(LncRNA)-Hox antisense RNA(HOTAIR)in patients with rheumatoid arthritis(RA)and to explore its clinical significance.Methods Totally 132 RA patients(RA group)and 50 healthy people(control group)were selected.The fasting venous blood of patients was taken on the second day of admission in the RA group and on admission in the control group.Serum lncRNA-HOTAIR was detected by real-time RT-PCR and anti-cyclic citrullinated peptide(CCP)antibody was detected by ELISA.RF and CRP were detected by rate nephelometry,and ESR by Widmanstatten method.The relationship between serum lncRNA-HOTAIR level and clinical parameters in RA patients was analyzed.ROC curve was used to analyze the diagnostic value of serum lncRNA-HOTAIR level in RA patients.Results The levels of lncRNA-HOTAIR,anti-CCP antibody,RF,CRP,and ESR in the RA group were higher than those in the control group(all P<0.05).The level of lncRNA-HOTAIR in patients with high disease activity was higher than that in patients with moderate and low activities(P<0.05).The level of lncRNA-HOTAIR in the RA group was positively correlated with DAS28 score(r=0.645,P=0.009).The area under ROC curve of serum lncRNA-HOTAIR was 0.706,with the sensitivity of 71%,and the specificity of 82%.Conclusion The lncRNA-HOTAIR level in patients with RA increases,which is related to disease activity,and has a higher accuracy in the diagnosis of RA.
