摘要
目的观察2型糖尿病下肢血管病变患者的血清脂肪因子血管生成素样蛋白4(ANGPTL4)水平,并分析其临床意义。方法回顾性选取淮安第一医院2016年8月至2018年8月接受治疗的2型糖尿病患者为研究对象,并根据其是否发生下肢血管病变分为合并下肢血管病变组和未合并下肢血管病变组,同时选取同期在该院接受体检的健康成年人作为对照。观察三组研究对象血清ANGPTL4水平,比较三组研究对象炎症因子[白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)]、血脂[胆固醇(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)]水平的差异,分析2型糖尿病下肢血管病变患者血清ANGPTL4水平与炎症因子和血脂水平的相关性。结果合并下肢病变组患者的ANGPTL4水平为(612.75±15.36)pg/ml,低于未合并下肢病变组[(801.39±12.02)pg/ml]和对照组[(945.26±20.13)pg/ml],差异有统计学意义(P<0.05);合并下肢病变组患者的IL-6、hs-CRP和TNF-α水平分别为(26.49±3.12)mg/L、(14.36±3.27)mg/L和(33.48±4.93)ng/L,高于未合并下肢病变组[(18.27±2.05)mg/L、(8.08±2.65)mg/L、(18.65±3.06)ng/L]和对照组[(8.65±1.64)mg/L、(6.14±1.20)mg/L、(10.32±2.03)ng/L],差异有统计学意义(P<0.05);合并下肢病变组患者的HDL-C水平为(0.86±0.13)mmol/L,低于未合并下肢病变组[(1.32±0.28)mmol/L]和对照组[(1.55±0.46)mmol/L],合并下肢病变组患者的TC、LDL-C和TG水平均高于未合并下肢病变组和对照组(P<0.05);2型糖尿病下肢病变患者的ANGPTL4水平与IL-6、hs-CRP、TNF-α、HDL-C水平负相关,与TC、LDL-C和TG水平正相关。结论2型糖尿病下肢血管病变患者的ANGPTL4水平较低,且与炎症因子水平和血脂水平密切相关。
Objective To observe the serum adipocytokinin-like protein 4(ANGPTL4)level in lower limb angiopathy in patients with type 2 diabetes mellitus and to analyze its clinical significance.Methods This Retrospective study enrolled patients with type 2 diabetes who were treated in our hospital from August to February 2018.The patients were divided into the lower limb angiopathy group and non-lower limb angiopathy group according to whether they had lower limb vascular disease.Healthy adults who were examined in our hospital during the same period were selected as controls.The serum angiopoietin-like protein 4 levels in the three groups were observed.The differences of inflammatory factors[interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)],and blood lipid[Cholesterol(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C)]levels among the three groups were compared.The correlation between serum angiopoietin-like protein 4 levels and inflammatory factors,blood lipid levels were analyzed in type 2 diabetes patients with lower limb angiopathy.Results The ANGPTL4 level in lower limb angiopathy group was(612.75±15.36)pg/ml,significantly lower than that in non-lower limb angiopathy group and control group(P<0.05).The levels of IL-6,CRP and TNF-αin lower limb angiopathy group were(26.49±3.12)mg/L,(14.36±3.27)mg/L and(33.48±4.93)ng/L,respectively,significantly higher than those in non-lower limb angiopathy group[(18.27±2.05)mg/L,(8.08±2.65)mg/L,(18.65±3.06)ng/L]and control group[(8.65±1.64)mg/L,(6.14±1.20)mg/L,(10.32±2.03)ng/L](P<0.05).The level of HDL-C in lower limb angiopathy group was 0.86±0.13 mmol/L,lower than that in non-lower limb angiopathy group[(1.32±0.28)mmol/L]and control group[(1.55±0.46)mmol/L].The levels of TC,LDL-C and TG in lower limb angiopathy group were higher than those in non-lower limb angiopathy group and control group.(P<0.05).The ANGPTL4 level in type 2 diabetes patients with lower limb angiopathy was negatively correlated with
