电针对乳腺癌术后镇痛和TNF-α、IL-6及免疫功能的影响 被引量:11

Analgesic Effect of Electroacupuncture After Surgery for Breast Cancer and Its Influence on TNF-a, IL-6 and Immune Function

摘要 目的观察电针对乳腺癌患者手术后镇痛效果,以及对肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)和免疫功能的影响。方法选取乳腺癌手术患者84例,随机分为观察组和对照组,每组42例。两组患者均采用静脉全身麻醉,观察组麻醉诱导前电针刺激内麻点和行间穴30 min,对照组患者术后采用静脉自控镇痛(PCIA)。记录两组患者电针刺激前(T0)、术后2 h(T1)、术后12 h(T2)、术后24 h(T3)和术后48 h(T4)各时间点的视觉模拟评分(VAS),在各时间点抽取静脉血测定TNF-α、IL-6以及CD3^﹢、CD4^﹢、CD8^﹢和CD4^﹢/CD8^﹢的含量。结果观察组VAS评分在T1与T0比较,明显升高(P<0.05);对照组在T1、T2、T3与T0比较,明显升高(P<0.05);观察组VAS评分在T1、T2、T3与对照组比较,显著下降(P<0.05)。对照组TNF-α和IL-6在T1、T2、T3与T0比较,显著升高(P<0.05);观察组TNF-α和IL-6在T1、T2、T3与对照组比较,显著下降(P<0.05)。观察组CD3^﹢、CD4^﹢和CD4^﹢/CD8^﹢在T1与T0比较,明显下降(P<0.05);对照组CD3^﹢、CD4^﹢和CD4^﹢/CD8^﹢在T1、T2、T3、T4与T0比较,明显下降(P<0.05);观察组CD3^﹢、CD4^﹢和CD4^﹢/CD8^﹢在T1、T2、T3、T4与对照组比较,显著升高(P<0.05)。结论乳腺癌根治术患者行术前电针预处理,能为患者提供良好的术后镇痛,降低应激反应,保护免疫功能。 Objective To observe the analgesic effect of electroacupuncture after surgery for breast cancer and its influence on tumor necrosis factor(TNF)-α,interleukin(IL)-6 and immune function.Method Eighty-four patients who were going to receive breast cancer surgery were randomized into an observation group and a control group,with 42 cases in each group.The two groups both received general intravenous anesthesia.The observation group was given electroacupuncture at Neimadian(Extra)and Xingjian(LR2)for 30 min prior to anesthesia,and the control group was intervened by patient-controlled intravenous analgesia(PCIA)after surgery.The Visual Analogue Scale(VAS)score was recorded before electroacupuncture stimulation(T0),2 h after surgery(T1),12 h after surgery(T2),24 h after surgery(T3)and 48 h after surgery(T4);the contents of TNF-α,IL-6,CD3^﹢,CD4^﹢,CD8^﹢and CD4^﹢/CD8^﹢in venous blood were detected at each time point.Result The VAS score increased significantly at T1 compared with that at T0 in the observation group(P<0.05);the VAS score increased significantly at T1,T2 and T3 compared with that at T0 in the control group(P<0.05);the VAS score in the observation group was markedly lower than that in the control group at T1,T2 and T3(P<0.05).The contents of TNF-α and IL-6 increased significantly at T1,T2 and T3 compared with those at T0 in the control group(P<0.05);the contents of TNF-α and IL-6 in the observation group were significantly lower than those in the control group at T1,T2 and T3(P<0.05).The contents of CD3^﹢,CD4^﹢and CD4^﹢/CD8^﹢decreased significantly at T1 compared with those at T0 in the observation group(P<0.05);the contents of CD3^﹢,CD4^﹢and CD4^﹢/CD8﹢decreased significantly at T1,T2,T3 and T4 compared with those at T0 in the control group(P<0.05);the contents of CD3^﹢,CD4^﹢and CD4^﹢/CD8^﹢in the observation group were significantly higher than those in the control group at T1,T2,T3 and T4(P<0.05).Conclusion Intervention with electroacupuncture prior to breast cancer sur
