摘要
目的:观察微创穴位埋线法治疗原发性三叉神经痛的临床效果及治疗前后卡马西平使用情况和不良反应的变化。方法:纳入被诊断为原发性三叉神经痛且口服卡马西平3个月以上的患者122例进行微创穴位埋线治疗,治疗后3个月进行疗效评定,并对比治疗前后患者使用卡马西平的用药频次、剂量和不良反应的改变情况。结果:122例患者中100例患者经穴位埋线治疗后疼痛减轻或消除,治疗有效率达81.97%。埋线治疗后有52例(42.62%)患者停药,每天用药频次不少于3次的患者减少了32例(P<0.05),用药频次2次/d的患者也减少了32例(P<0.05),用药剂量不少于900 mg/d的患者减少了10例(P<0.05),用药剂量为300~900 mg/d的患者减少了42例(P<0.05)。治疗前因口服卡马西平出现不良反应的有52例(42.62%),治疗后减少至18例(14.75%)。结论:微创穴位埋线法治疗三叉神经痛简单而且有效,可降低患者对卡马西平的依赖,减少不良反应。
Objective: To observe the clinical effects of minimally invasive acupoint thread-embedding in the treatment of primary trigeminal neuralgia and the changes of carbamazepine usage and adverse drug reactions after the thread-embedding. Methods: 122 patients with primary trigeminal neuralgia and long-term oral administration of carbamazepine were treated with minimally invasive acupoint thread-embedding. The taking frequency, dose and adverse reactions of carbamazepine before and after the treatment were analyzed 3 months after treatment. The changes of carbamazepine medication and adverse reactions were observed. Results: The effective rate of acupoint thread-embedding was 81.97% in 122 patients. 52(42.62%) patients stopped carbamazepine use after the treatment(P<0.05), 32 patients who had used the drug at least 3 times a day decreased the drug use times(P<0.05), so did another 22 patients had used the drug 2 times a day(P<0.05). Dosage was reduced in 10 patients who had used the dose of at least 900 mg/d(P<0.05);the number of patients with 300-900 mg/d decreased from 66 cases to 24 cases(P<0.05). There were 52 cases(42.62%) of adverse reactions caused by carbamazepine before thread-embedding, and the case of adverse reactions decreased to 18 cases(14.75%) after the thread-embedding. Conclusion: Minimally invasive acupoint thread-embedding is not only effective in the treatment of primary trigeminal neuralgia, but also can significantly reduce the adverse reactions of carbamazepine.
