摘要 目的分析探讨3D打印辅助技术对主动脉夹层治疗方案制定和效果的影响。方法回顾性分析2017年1月至2018年3月在临汾市人民医院诊治主动脉夹层的患者20例,其中男14例,女6例,年龄25~58(37.2±8.2)岁。所有患者经主动脉CT血管造影(CTA)确诊为主动脉夹层,均行主动脉3D实体打印,依据主动脉CTA和3D打印模型相结合制订手术方案,并全部进行手术治疗。结果完成手术20例患者,术后1例患者死亡。患者术前近端破口真腔直径(21.4±1.4)mm,术后1个月(24.2±1.3)mm;术前气管分叉真腔直径(21.2±0.8)mm,术后1个月(23.9±1.2)mm,两者术后均较术前有明显扩大,差异均有统计学意义(均P<0.05)。术前腹腔干水平真腔直径(13.8±2.1)mm,术后1个月(14.1±1.4)mm,术前肾动脉水平真腔直径(11.3±2.3)mm,术后1个月(11.5±1.4)mm,术后较术前均无明显扩大,差异均无统计学意义(均P>0.05)。结论 3D打印能够真实还原病变主动脉形态,可以直观、立体地了解病变,对病理分型、术前数据测量、手术规划起到重要作用。 Objective To analyze the influence of 3 D printing assistive technique on the development and effect of aortic dissection therapy strategy. Methods A retrospective analysis of 20 patients with aortic dissection in our hospital from January 2017 to February 2018 including 14 males and 6 females, who were 25–58(37.2±8.2)years old with a mean age of 37.2 years. All patients were diagnosed with aortic dissection based on CTA,and the aorta was printed with 3 D printing. The surgical plan was established according to the combination of aorta CTA and 3 D printed model,and all surgical procedure were performed. Results A total of 20 cases of surgery were completed, and 1 case died after operation. The diameter of the proximal cavity of the patient before surgery was(21.4±1.4)mm and post-operation was(24.2±1.3)mm, the diameter on tracheal bifurcation level was(21.2±0.8)mm and post-operation was(23.9±1.2)mm, with a statistically significant difference(P<0.05). The diameter on abdominal cavity level before surgery was(13.8±2.1)mm and postoperation was(14.1±1.4)mm, on renal artery level before surgery was(11.3±2.3)mm and post-operation was(11.5±1.4)mm,with no significant expansion(P>0.05). Conclusions 3 D printing can reduce the shape of the diseased aorta, and help to understand the lesions intuitively and stereoscopically, which plays an important role in pathological classification, preoperative data measurement and surgical planning.
