摘要
目的:探讨关节镜下外侧支持带松解联合内侧髌股韧带(MPFL)重建治疗复发性髌骨脱位的临床效果。方法:选取2015年1月-2017年12月本院新诊断复发性髌骨脱位患者60例,按随机数字表法分为观察组和对照组,每组30例。观察组行关节镜下外侧支持带松解和MPFL重建,对照组行常规手术下外侧支持带松解联合MPFL重建。观察两组基线期与治疗后髌骨CT测量值和髌股关节功能评分变化。结果:两组基线期股骨滑车角、和谐角比较,差异均无统计学意义(P>0.05);观察组治疗后股骨滑车角、和谐角均显著低于对照组(P<0.05)。两组基线期Lysholm积分、Kujala积分比较,差异均无统计学意义(P>0.05);观察组治疗后Lysholm积分、Kujala积分均显著高于对照组(P<0.05)。结论:与常规手术下外侧支持带松解联合MPFL重建相比,关节镜下外侧支持带松解和MPFL重建治疗复发性髌骨脱位效果更佳。
Objective: To discuss the clinical effect of arthroscopic lateral retinacular release combined with medial patellofemoral ligament reconstruction for recurrent patellar dislocation. Method: A total of 60 patients with recurrent patellar dislocation treated in our hospital from January 2015 to December 2017 were selected, and according to the random indicator method they were divided into the observation group and the control group, 30 cases in each group. The control group received lateral retinacular release combined with medial patellofemoral ligament reconstruction, the observation group received arthroscopic lateral retinacular release combined with medial patellofemoral ligament reconstruction. The changes of patellar CT measurements and patellofemoral joint function scores at baseline and after treatment of two groups were observed. Result: Baseline period, the scores of Lysholm and Kujala, femoral trochlear angle, lateral patellofemoral angle of two groups were compared, there were no statistical significance(P>0.05). After treatment, femoral trochlear angle and lateral patellofemoral angle of the observation group were lower than those of the control group(P<0.05). There were no significant differences in Lysholm score and Kujala score between the two groups at baseline(P>0.05). After treatment, the scores of Lysholm and Kujalain of the observation group were higher than those of the control group(P<0.05). Conclusion: Compared with lateral retinacular release combined with medial patellofemoral ligament reconstruction, the effect of arthroscopic lateral retinacular release combined with medial patellofemoral ligament reconstruction treatment on medial patellofemoral ligament reconstruction for recurrent is better.
