摘要
目的分析乳腺良恶性肿块X线的边缘征象,提高对肿块的良恶性鉴别诊断水平。方法收集本院2012—2018年经手术病理证实的乳腺肿块130例,对于一侧乳腺内多发肿块,取最大的1枚作为研究对象,良恶性肿块各65例。结果良性肿块中,边缘清晰39例,占60%,边缘模糊16例,占25%,边缘小分叶3例,占5%,边缘浸润5例,占8%,星芒状2例,占3%;恶性肿块中,边缘清晰6例,占9%,边缘模糊6例,占9%,边缘小分叶13例,占20%,边缘浸润28例,占43%,星芒状12例,占18%;把边缘清晰和模糊归为良性征象,把小分叶、边缘浸润和星芒状归为恶性征象,良性肿瘤中边缘清晰和模糊者55例,占85%,而恶性肿瘤中边缘浸润、小分叶和星芒状者53例,占82%,二者之间的X线表现存在着显著的统计学差异P<0.05。结论良恶性肿块的边缘征象有一定的特征性,肿块边缘征象对判断肿块性质具有重要的价值,结合临床触诊,可提高对肿块的良恶性鉴别的诊断水平。
Objective The marginal signs of the X-ray ofbreast tumors were compared and analyzed to improve the differential diagnosis of benign and malignant masses.Methods One hundred and thirty cases of breast mass confirmed by operation and pathology in our hospital from 2012 to 2018 were collected.For the multiple masses in one breast,the largest one was taken as the research object,and 65 cases of benign and 65 cases of malignant masses were used.Results Among the benign masses,39 cases were clear,accounting for 60%,16 cases were blurred,accounting for 25%,3 cases were small lobes,accounting for 5%,5 cases were infiltrating,accounting for 8%,and 2 cases were star-shaped,accounting for 3%;In the malignant mass,6 cases were clear,accounting for 9%,6 cases were blurred,accounting for 9%,13 cases were small lobes,accounting for 20%,28 cases were infiltrating,accounting for 43%,and 12 cases were star-shaped,accounting for 18%;Clear and blurred are classified as benign signs,and small lobes,infiltrates,and star-shaped are classified as malignant signs.In the benign tumors,55 cases are clear and blurred,accounting for 85%,while in malignant tumors,53 cases are marginal infiltration,small lobes and star-shaped in malignant tumors were of scorpion,accounting for 82%.There was a significant statistical difference between the two groups(P<0.05).Conclusion The marginal signs of benign and malignant masses have certain characteristics.The signs of edge of the mass have important value in judging the nature of the mass.Combined with clinical palpation,the differential diagnosis level of benign and malignant tumors can be improved.
