摘要
目的观察通腑清营汤对脓毒症急性肺损伤大鼠Fas和肿瘤坏死因子(TNF)-α水平的影响,探讨其治疗脓毒症急性肺损伤的作用机制。方法清洁级健康成年雄性SD大鼠随机分为模型组、中药组和地塞米松组,每组再分4个亚组(12、24、48、72 h时间点)观察,每组6只,共72只。各给药组给予相应药液灌胃,模型组给予生理盐水灌胃,每日1次,连续3 d。末次给药2 h后采用盲肠结扎穿孔术制作脓毒症急性肺损伤模型,分别于术后12、24、48、72h处死大鼠,观察大鼠一般情况;取大鼠肺组织称重,测定大鼠肺组织湿/干重比值(W/D),HE染色观察肺组织病理变化,TUNEL法检测肺组织细胞凋亡指数(AI),Western blot检测肺组织Fas蛋白表达量;采集肺泡灌洗液标本,ELISA检测TNF-α含量。结果中药组、地塞米松组术后大多时间点W/D值和AI均低于模型组(P<0.05,P<0.01),中药组与地塞米松组比较差异无统计学意义(P>0.05);中药组和地塞米松组肺泡灌洗液TNF-α含量在术后不同时间点均较模型组显著降低(P<0.01),中药组肺泡灌洗液TNF-α含量术后各时间点较地塞米松组明显增加(P<0.01);中药组和地塞米松组肺组织Fas蛋白表达在术后48、72 h较模型组明显降低(P<0.05),地塞米松组Fas蛋白表达术后48 h低于中药组(P<0.05)。结论通腑清营汤具有减轻脓毒症急性肺损伤大鼠炎症反应及抗凋亡的作用,其机制与下调模型大鼠肺组织Fas蛋白的表达、抑制TNF-α炎症介质的水平相关。
Objective To observe the effects of Tongfu Qingying Decoction on Fas protein and TNF-α in rats with acute lung injury induced by sepsis;To discuss the mechanism of Tongfu Qingying Decoction in treating acute lung injury by sepsis. Methods Clean healthy adult male SD rats were randomly divided into model group, TCM group and dexamethasone group, each group was divided into four subgroups(12 h, 24 h, 48 h and 72 h), with 6 cases in each group, a total of 72 rats. Each administration group was given relevant medicine for gavage, and model group was given saline for gavage, once a day for 3 d. Acute lung injury model of sepsis was prepared by cecal ligation and perforation after 2 h of last administration. Rats were sacrificed at 12, 24, 48, 72 h after operation and the general condition of rats were observed. Rat lung tissue was taken and weighed to determine the wet/dry weight ratio(W/D) of lung tissue. HE staining was used to observe the pathological changes of lung tissue. TUNEL method was used to detect the apoptosis index(AI) of lung tissue. The amount of Fas protein expression in lung tissue was detected by Western blot. The alveolar lavage fluid samples were collected and the TNF-α content was detected by ELISA. Results Compare with the model group, W/D values and AI of TCM group and dexamethasone group were lower at most postoperative time points(P<0.05, P<0.01), but there was no statistical significance in W/D values and AI between TCM group and dexamethasone group(P>0.05). The content of TNF-α in alveolar lavage fluid in TCM group and dexamethasone group was significantly lower than that in the model group at different postoperative time points(P<0.01). The content of TNF-α in alveolar lavage fluid in TCM group was higher than that in the dexamethasone group at each postoperative time point(P<0.01). The expression of Fas protein in lung tissues of TCM group and dexamethasone group was lower than that of the model group at 48 h and 72 h after operation(P<0.05), and the expression of Fas protein in dexam
