摘要
目的了解HIV感染和艾滋病合并结核病(以下简称HIV/TB)患者的综合特征,为HIV/TB的诊断提供参考。方法选取2014年9月-2018年8月在鹤壁市传染病医院就诊的50例HIV/TB患者作为研究组,并随机抽取同期就诊的63例TB患者作为对照组。通过对患者的流行病学、临床、影像学等指标进行分析,了解HIV/TB患者的综合特征。结果HIV/TB组的结核病确诊时间(28.5天)长于TB组(18.0天)。咳痰率(22.0%)低于TB组(42.9%),其它症状的发生率两组相接近;左下肺病变(26%)较TB组患者(4.8%)多,而右上、中、下肺病变相对少,空洞和胸膜肥厚的发生率(4.0%和2.0%)低于TB组(20.6%和15.9%),淋巴结增大的发生率(18.0%)高于TB组(4.8%);两组细菌学阳性率均较低,在3%-17.5%之间;PPD和IGRAs的阳性率(18.0%和46.0%)低于TB组(69.8%和66.7%);CD^+4细胞、CD^+4/CD^+8比值、白蛋白、白细胞计数、淋巴细胞计数和中性粒细胞计数均低于TB组。结论HIV/TB与TB患者有不同的综合特征,了解此特征,对HIV/TB的诊断具有一定的辅助价值。
Objective To understand the comprehensive characteristics of HIV infection and AIDS combined with tuberculosis(HIV/TB)patients,so as to provide reference for the diagnosis of HIV/TB.Methods 50 cases of HIV/TB patients admitted to Hebi hospital of infectious diseases from September 2014 to August 2018 were selected as the study group,and 63 cases of TB patients admitted during the same period were randomly selected as the control group.Their epidemiological,clinical and imaging characteristics of HIV/TB patients were analyzed to understand the comprehensive characteristics.Results The time of TB diagnosis in the HIV/TB group(28.5 days)was longer than the TB group(18.0 days).The expectoration rate(22.0%)was lower in the HIV/TB group than in the TB group(42.9%).The incidence of cavity and pleural hypertrophy(4.0%and 2.0%)was lower in the HIV/TB group than in the TB group(20.6%and 15.9%),and the incidence of lymph node enlargement(18.0%)was higher in the HIV/TB group than in the TB group(4.8%).The positive rate of bacteriology was low in both groups,between 3%and 17.5%.The positive rate of PPD and IGRAs(18.0%and 46.0%)was lower in the HIV/TB group than in the TB group(69.8%and 66.7%).CD^+4 cells,+4/CD^+8 ratio,albumin,white blood cell count,lymphocyte count and neutrophil count were lower in the HIV/TB group than in the TB group.Conclusion There are different comprehensive characteristics between HIV/TB patients and TB patients.Understanding these characteristics has certain auxiliary value for the diagnosis of HIV/TB.
