摘要
目的通过分析牙龈卟啉单胞菌(Porphyromonas gingivalis,P.gingivalis)kgp基因型在原发性根尖周感染患牙根管中的分布情况,了解kgp基因在原发性根尖周感染中的基因多态性,并分析不同基因型与临床症状的相关性。方法临床收集190例原发性根尖周感染患者,采集感染根管细菌学样本,抽提DNA,使用P.gingivalis的16S rRNA基因的特异引物PCR检测样本中是否有P.gingivalis,然后针对P.gingivalis检出阳性样本,再使用kgp特异引物PCR获得kgp催化域片段,通过TruⅠ酶切kgp催化域片段确定kgp的基因型,并使用Pearson χ^2方法分析所得到的实验数据。结果在采集的190例原发性根尖周感染样本中,共检出P.gingivalis 55例,其中kgpⅠ型检出率为27.3%,kgpⅡ型检出率为38.2%。P.gingivalis的检出与临床症状有相关性;kgpⅠ型与疼痛具有相关性。结论原发性根尖周感染的根管中P.gingivalis kgp基因存在基因多态性。因为有些样本中P.gingivalis的量较少,该研究中kgp特异引物PCR的方法难以检测到kgp基因,所以今后需发展更灵敏的检测手段以对感染根管中kgp基因做进一步的研究。
Objective To investigate the gene polymorphism of kgp gene in primary periapical infection root canal through the distribution of Porphyromonas gingivalis and Porphyromonas gingivalis’two kgp gene types,and to analyze the correlation between the genotypes and clinical symptoms.Methods One hundred and ninety cases of primary periapical infection patients in clinic were collected.The samples from their infected root canal were collected and DNA was extracted.Polymerase Chain Reaction was used to determine the presence of Porphyromonas gingivalis.The kgp catalytic domain with the help of Polymerase Chain Reaction was got.TruⅠenzyme digestion to the kgp catalytic domain was used to determine the Porphyromonas gingivalis gene polymorphism.After all,statistical methods were used to analyze the experimental data.Results Fifty-five patients in 190 patients with primary periapical infection could be detected with Porphyromonas gingivalis.The detection rate of kgpⅠtype was 27.3%,while the detection rate of kgpⅡtype was 38.2%.Porphyromonas gingivalis was correlated with clinical symptoms.kgp I type was correlated with pain.Conclusion The kgp gene of Porphyromonas gingivalis which is in root canal of primary periapical infection has gene polymorphism.Because some samples do not contain enough quantity of Porphyromonas gingivalis,it is sometimes difficult to use kgp specific primers PCR to detect kgp gene.In further research,more sensitive means should be found to detect kgp gene in infected root canals.
