摘要
目的分析诊断为含薄壁囊腔周围型肺癌的CT影像表现,提高对此类疾病的认识及诊断准确率。方法回顾性分析16例经病理证实的含薄壁囊腔周围型肺癌的CT征象,总结含薄壁囊腔周围型肺癌的CT表现特点。结果本组16例患者中,15例为腺癌,1例为腺鳞癌;其中病变位于肺上叶11例,肺下叶4例;右肺中叶1例;10例为多发囊腔,6例为单发囊腔;囊腔形态呈圆形或类圆形14例,不规则形2例;12例囊腔内有分隔,4例囊腔内无分隔;12例有分叶、毛刺、胸膜凹陷征,4例无分叶、毛刺、胸膜凹陷征;7例囊腔周围伴有磨玻璃密度影,9例囊腔周围伴实性结节。结论含薄壁囊腔周围型肺癌的CT表现有一定特征性,综合分析囊腔的形态及周围征象,有利于提高诊断准确率。
Objective To analyze the CT manifestations of parenchymal peripheral type lung cancer and improve the recognition and early diagnosis rate of this disease.Methods CT findings of 16 cases with parenchymal lung cancer confirmed by pathology were retrospectively analyzed.To summarize the CT features of parenchymal periluminal lung cancer.Results Pathology confirmed thaT15 cases were adenocarcinoma.One case was adenosquamous carcinoma.The lesions were located in the upper lobe in 11 cases and the lower lobe in 4 cases.Middle lobe of right lung in 1 case was shown.There were 10 cases with multiple cysts,while 6 cases were single hair cysts.The shape of the cysts was round/almost round in 14 cases and irregular in 2 cases.In 12 cases,there was space within the cystic cavity.In 4 cases,there was no space within the cystic cavity.There were leaf,burr and pleural depression in 12 cases,and no leaf,burr and pleural depression in 4 cases.Density shadow of ground glass was observed around the cysts in 7 patients,and solid nodules were observed around the cysts in 9 patients.Conclusion CT findings of peripheral lung cancer associated with thin-walled airspace have certain characteristics.The comprehensive analysis of cystic morphology and peripheral signs is helpful to improve the diagnostic rate.
