摘要
国家二孩政策的出台,使高龄孕产妇比例急剧增长。高龄可增加妊娠期糖尿病、妊娠期高血压疾病、胎盘异常、早产等并发症风险;高龄妊娠对胎儿有一定的影响,可增加出生缺陷、流产、死胎、巨大儿、胎儿生长受限的发生率;高龄对分娩方式也有一定的影响,可使剖宫产率增加;高龄妊娠增加了不良妊娠结局风险,如子宫破裂、产后出血,也可增加产后抑郁发生的风险。临床应对高龄孕产妇,加强孕前教育及优生健康检查、加强围生期保健与监护、规范产前检查,全面评估高危妊娠孕产妇的风险因素,及时发现和处理孕前、孕期及生产时的异常情况,确保母婴安全。
The promulgation of the national second child policy has dramatically increased the proportion of elderly pregnant women. Older age can increase the risk of pregnancy complications such as gestational diabetes mellitus,pregnancy-induced hypertension,abnormal placenta,premature delivery,etc. Older pregnancy has a certain impact on the fetus,which can increase the incidence of birth defects,abortion,stillbirth,macrosomia and fetal growth restriction;Older age has a certain impact on the mode of delivery,which can increase the rate of cesarean section;Older pregnancy increases the risk of adverse pregnancy outcomes,such as uterine rupture and postpartum hemorrhage,but also can increase the risk of postpartum depression. We should strengthen prenatal education and prenatal health examination,strengthen perinatal health care and monitoring,standardize prenatal examination,comprehensively assess the risk factors of high-risk pregnant women,timely detect and deal with the abnormal situation before,during pregnancy and parturition to ensure the safety of mothers and infants.
