摘要
目的 探讨子宫动脉上行支结扎联合子宫背带式缝扎术治疗产后出血的临床效果.方法 回顾性分析我院2017年1月至2019年3月收治的40例产后出血患者的临床资料,根据止血方法的不同将患者分为对照组(20例,传统纱条填塞法)和观察组(20例,子宫动脉上行支结扎+子宫背带式缝扎术).比较两组患者的临床疗效.结果 观察组的止血有效率明显高于对照组,子宫切除率、中转ICU率及并发症总发生率均明显低于对照组(P<0.05).治疗后,两组的卵巢功能、血常规各项指标水平均明显降低,但观察组明显高于对照组(P<0.05).治疗后,两组的CRP、IL-6水平均明显降低,且观察组明显低于对照组(P<0.05).结论 子宫动脉上行支结扎联合子宫背带式缝扎术用于产后出血患者治疗中具有良好的止血效果,可减少并发症,减轻对患者卵巢功能、血常规的影响,有利于抑制炎症反应.
Objective To explore the clinical effect of ligation of ascending branch of uterine artery combined with suture of uterine dorsal band in the treatment of postpartum hemorrhage. Methods The clinical data of 40 cases of postpartum hemorrhage from January 2017 to March 2019 were retrospectively analyzed,and the patients were divided into control group(20 cases, traditional gauze packing method) and observation group(20 cases, ligation of ascending branch of uterine artery+suture of uterine dorsal band) according to the different hemostasis methods. The clinical effects of the two groups were compared. Results The effective rate of hemostasis in the observation group was significantly higher than that in the control group, the rates of hysterectomy and conversion to ICU and the total incidence of complications in the observation group were significantly lower than those in the control group(P<0.05). After treatment, the levels of ovarian function and blood routine indexes in the two groups significantly decreased, but those in the observation group were significantly higher than the control group(P <0.05). After treatment, the levels of CRP and IL-6 in the two groups significantly decreased, and those in the observation group were significantly lower than the control group(P <0.05).Conclusion The ligation of ascending branch of uterine artery combined with suture of uterine dorsal band has a good hemostatic effect in the treatment of postpartum hemorrhage, it can reduce complications, alleviate the influences on ovarian function and blood routine, which is beneficial to inhibit the inflammatory reaction.
