摘要
目的:探讨球囊扩张术辅以口腔综合感觉训练对脑卒中后吞咽功能障碍的影响。方法:选取脑卒中患者60例,随机分为2组,各30例。对照组采用口腔综合感觉训练,观察组在对照组基础上,采用球囊扩张术,治疗4周后比较2组标准吞咽功能评定量表评分(SSA)、吞咽X线荧光透视检查评分(VFSS)、洼田饮水试验和临床疗效。结果:治疗前,2组SSA评分、VFSS评分比较无统计学意义(P>0.05);治疗后,与对照组比较,观察组SSA评分[(23.41±2.63)分vs(27.06±2.67)分]、VFSS评分[(6.33±1.10)分vs(4.30±1.06)分]均优于对照组(P<0.05),观察组洼田饮水试验分级均优于对照组(P<0.05);观察组总有效率(96.67%)高于对照组(73.33%)(P<0.05)。结论:球囊扩张术辅以口腔综合感觉训练可改善脑卒中患者吞咽功能障碍,恢复其吞咽功能。
Objective: To investigate the effect of balloon dilatation combined with oral comprehensive sensory training on swallowing dysfunction after stroke. Methods: Sixty patients with stroke were selected and divided into two groups according to double-blind method, 30 cases each. In the control group, oral comprehensive sensory training was used. On this basis, the experimental group was treated with balloon dilatation. After 4 weeks of treatment, the clinical efficacy of the two groups, the Putian drinking test, the standard swallowing function rating scale(SSA), Fluoroscopy score(VFSS) and the swallowing X-ray were compared. Results: Before treatment, there was no statistically significant difference between the two groups(P>0.05). After treatment, compared with the control group, the SSA score of the experimental group [(23.41±2.63) vs(27.06±2.67)], VFSS score [(6.33±1.10) vs(4.30±1.06)], the difference was statistically significant(P<0.05);After treatment, the grading of the drinking water test in the experimental group was better than the control group, and the difference was statistically significant(P<0.05). The total effective rate of the experimental group(96.67%) was higher than that of the control group(73.33%), and the difference was statistically significant(P<0.05). Conclusion: Balloon dilatation combined with oral comprehensive sensory training can improve swallowing dysfunction and restore swallowing function in stroke patients.
