摘要
目的:观察活血生骨汤联合玻璃酸钠关节腔内注射治疗膝骨关节炎的临床疗效。方法:选取在本院就诊的102例膝骨关节炎患者,按照随机数字表法分为对照组和研究组各51例。对照组给予玻璃酸钠注射液关节腔内注射治疗,研究组采用活血生骨汤联合玻璃酸钠关节腔内注射治疗,均连续治疗5周。比较2组临床疗效、疼痛程度、膝关节功能和炎症细胞因子水平。结果:研究组总有效率96.08%,高于对照组的78.43%,差异有统计学意义(P<0.05)。治疗后,2组视觉模拟评分法(VAS)评分均较治疗前降低(P<0.05),研究组VAS评分比对照组降低更明显(P<0.05)。治疗后,2组疼痛、功能、活动度、屈曲畸形、稳定性评分及总分均较治疗前升高(P<0.05);研究组上述5项细则评分及总分均高于对照组(P<0.05)。治疗后,2组白细胞介素-1β(IL-1β)和肿瘤坏死因子-α(TNF-α)水平均较治疗前降低(P<0.05),研究组IL-1β、TNF-α水平均比对照组下降更明显(P<0.05)。结论:活血生骨汤联合玻璃酸钠关节腔内注射治疗膝骨关节炎,可以减轻患者的疼痛程度,抑制IL-1β、TNF-α表达,提高膝关节功能。
Objective:To observe the clinical effect of Huoxue Shenggu Tang combined with sodium hyaluronate intra-articular injection for knee osteoarthritis.Methods:A total of 102 patients with knee osteoarthritis were selected and divided into the control group and the study group according to random number table method.The control group was treated with intra-articular injection of sodium hyaluronate,and the study group was treated with Huoxue Shenggu Tang combined with sodium hyaluronate intra-articular injection.Both groups were treated for five weeks.The clinical effect,the pain degree,the knee joint function and the inflammatory cytokines were compared in the two groups.Results:The total effective rate was 96.08% in the study group,higher than that of 78.43% in the control group,the difference being significant(P<0.05).After treatment,the scores of Visual Analogue Scale(VAS) in both groups were significantly decreased when compared with those before treatment(P<0.05),and the VAS score in the study group were decreased more significantly than those in the control group(P<0.05).After treatment,the scores of pain,function,activity,deformity,stability and the total score were all increased in the two groups when compared with those before treatment(P<0.05);the above five scores and the total score in the study group were higher than those in the control group(P<0.05).After treatment,the levels of interleukin-1β(IL-1β) and tumor necrosis factor-α(TNF-α) in both groups were decreased when compared with those before treatment(P<0.05),and the levels of IL-1β and TNF-α in the study group were decreased more significantly than those in the control group(P<0.05).Conclusion:The therapy of Huoxue Shenggu Tang combined with intra-articular injection of sodium hyaluronate for knee osteoarthritis,can reduce pain,inhibit the expression of IL-1β and TNF-α,and improve knee joint function.
