摘要
目的分析门诊抽血室不合格血标本的主要原因,并提出相应对策。方法收集2018年1~6月我院门诊部抽血室血标本623616份,由检验技师依靠肉眼和仪器判断血标本是否合格,反馈给抽血室后对不合格血标本逐一登记,重新抽血后再次送检。记录不合格血标本数量,分析不合格血标本产生原因和检验项目构成。结果623616份血标本中不合格标本为339份,不合格率为0.054%。339份不合格血标本原因构成从高到低依次为送错实验室58例(17.1%)、条码贴错导致血标本与检测项目不相符56例(16.5%)、试管错误(抗凝试管和普通试管用错;不同抗凝管混用)55例(16.2%)、血量过多42例(12.4%)、同名同条码标本39例(11.5%)、抗凝血液凝固或有细小凝块35例(10.3%)、血量不足31例(9.1%)、溶血14例(4.1%)、脂浊9例(2.7%)。不合格血标本项目构成由高到低依次为红细胞沉降率64例(0.267%)、凝血功能75例(0.204%)、血常规92例(0.059%)、其他项目57例(0.028%)、生化51例(0.025%)。结论血标本不合格因素涉及护士、医生、检验师、护理员、护工等诸多环节。认真处理好每一个环节,提高血标本质量,给医生诊断提供依据,促进患者早日康复。
Objective To analyze the main reasons of unqualified blood samples in outpatient blood sampling room and put forward corresponding countermeasures.Methods A total of 623616 blood samples from January to June 2018 were collected in the blood sampling room of the outpatient department of our hospital,the inspection technician relied on the naked eye and instruments to determine whether the blood samples were qualified,after feedback to the blood sampling room,the unqualified blood samples were registered one by one,and then sent for inspection again after the blood sampling.The number of unqualified blood samples were recorded,the causes of unqualified blood samples and the composition of inspection items were analyzed.Results Of 623616 blood samples,339 samples were unqualified,the unqualified rate was 0.054%.The causes of 339 unqualified blood samples were 58(17.1%)sent to the wrong laboratory,56(16.5%)caused by the wrong barcode,55(16.2%)caused by the wrong test tube(anticoagulant tube and common tube;mixed use of different anticoagulant tubes),42(12.4%)with excessive blood volume,39(11.5%)with the same barcode,anticoagulant blood coagulation or There were 35 cases of small clots(10.3%),31 cases of insufficient blood volume(9.1%),14 cases of hemolysis(4.1%),9 cases of lipid turbidity(2.7%)from high to low.From high to low,the items of unqualified blood samples were 64 cases(0.267%),75 cases(0.204%),92 cases(0.059%),57 cases(0.028%)and 51 cases(0.025%).Conclusion The unqualified factors of blood sample involve nurses,doctors,examiners,nurses,nurses and so on.Carefully handle every link,improve the quality of blood samples,provide basis for doctors′diagnosis,and promote patients′early recovery.
