稳定性冠心病合并2型糖尿病患者冠状动脉病变程度与HbA1c水平相关性分析 被引量:15

摘要 目的探讨稳定性冠状动脉粥样硬化性心脏病(冠心病)合并2型糖尿病(T2DM)患者冠状动脉(冠脉)病变程度与糖化血红蛋白(HbA1c)水平的相关性。方法回顾性分析2016年1月至2018年1月于莱芜市人民医院心内科诊治的131例稳定性冠心病合并T2DM患者临床资料,根据SYNTAX积分将患者分为轻度病变组(n=53例)、中度病变组(n=42例)和重度病变组(n=36例)。对比各组HbA1c等生化指标的差异。Logistic回归分析HbA1c水平与稳定性冠心病合并T2DM患者SYNTAX积分相关性。结果重度病变组患者年龄偏大,冠心病病程偏长,高脂血症和陈旧性心肌梗死病史比例偏高(P<0.05),随着冠脉病变程度的加重,血清HbA1c、SYNTAX积分逐渐增高(P<0.05)。相关性分析显示HbA1c水平与SYNTAX积分呈正相关(r=0.663,P<0.05),多元Logistic回归分析示HbA1c与SYNTAX积分独立相关(β=1.602,OR=4.963,P<0.001)。结论 HbA1c与稳定性冠心病合并T2DM患者冠脉SYNTAX积分有关,是影响其冠脉病变程度的重要因素,临床可用于预测稳定性冠心病合并T2DM患者病情。 Objective To investigate the correlation between severity of coronary artery lesions and level of glycated hemoglobin(HbAlc) in patients with stable coronary heart disease(CHD) complicated by type 2 diabetes mellitus(T2DM). Methods The clinical materials from the patients(n=131) with stable CHD complicated by T2DM, chosen from Department of Cardiology in People’s Hospital of Laiwu City from Jan. 2016 to Jan. 2018, were analyzed retrospectively. All patients were divided, according to SYNTAX integrals, into mild lesion group(n=53), moderate lesion group(n=42) and severe lesion group(n=36). The difference in biochemistry indexes including HbA1c were compared among all groups. The correlation between HbA1c level and SYNTAX integrals was analyzed by using Logistic regression analysis. Results The patients were older with longer CHD course, and the percentages of cases of hyperlipidemia and old myocardial infarction(OMI) were higher in severe lesion group(P<0.05). The level of HbA1c and SYNTAX integrals increased along with the exacerbation of coronary lesions(P<0.05). The correlation analysis showed that HbA1c level was positively correlated to SYNTAX integrals(r=0.663, P<0.05), and multi-factor Logistic regression analysis showed that HbA1c level were independently correlated to SYNTAX integrals(β=1.602, OR=4.963, P<0.001). Conclusion HbA1c level is correlated to SYNTAX integrals, which are important factors influencing the severity of coronary lesions and can be taken to predict disease state in patients with stable CHD complicated by T2DM.
