摘要
目的探讨改良根治性颈清扫术与颈侧清扫术对喉癌声门上型颈淋巴结病理阴性患者的疗效。方法选取2012年1月至2016年12月间陕西省雁塔区中医医院收治的79例喉癌患者,根据清扫术类型不同分组,采用颈侧清扫术治疗的38例患者纳入观察组,采用改良根治性颈清扫术治疗的41例患者纳入对照组。比较两组患者的围术期指标、治疗效果、术后并发症发生率及随访2年的淋巴结转移率。结果观察组患者术中出血量及住院时间均低于对照组,差异均有统计学意义(P<0.05)。观察组患者治疗优良率为97.4%,高于对照组的78.0%,差异有统计学意义(P<0.05)。观察组患者术后并发症发生率为7.9%,低于对照组的39.0%,差异有统计学意义(P<0.05)。观察组患者术后2年无淋巴结转移,对照组患者术后2年有1例发生淋巴结转移,转移率为2.4%,组间比较,差异无统计学意义(P>0.05)。结论与改良根治性颈清扫术相比,采用颈侧清扫术可提高喉癌声门上型颈淋巴结病理阴性患者的治疗效果,减少术中出血量,缩短住院时间,降低术后并发症发生率,值得临床推广应用。
Objective To investigate the efficacy of modified radical cervical dissection and lateral cervical dissection in pathologically negative cervical lymph node in supraglottic laryngeal cancer.Methods Seventy-nine patients with laryngeal cancer treated at Yanta District Hospital of Traditional Chinese Medicine from January 2012 to December 2016 were retrospectively selected and divided into an observation group(38 patients)and a control group(41 patients)according to different dissection types.Patients in the observation group were given lateral cervical dissection and poatients in the control group were given modified radical cervical dissection.The perioperative indicators,treatment efficacy,postoperative complication rate and lymph node metastasis rate were compared between the two groups.Results The intraoperative blood loss of blood and the length of hospital stay were higher in the observation group than in the control group(all P<0.05).The excellent rates was 97.4%in the observation group,which was higher than 78.0%of the control group(P<0.05).The incidence of postoperative complications was 7.9%which was lower than 39.0%of the control group(P<0.05).No lymph node metastasis occurred in the observation group at 2 years after the surgery and lymph node metastasis occurred in 1 patient(2.4%)in the control group(P>0.05).Conclusion Compared with the modified radical cervical dissection,the lateral cervical dissection could improve the treatment efficacy,and reduce the intraoperative blood loss,length of hospital stay and incidence of postoperative complications pathologically negative cervical lymph node in supraglottic laryngeal cancer,which was worthy of clinical promotion and application.
