摘要
目的分析评价在慢性肾衰合并重症感染中应用高通血液透析对其炎性介质的清除作用。方法本次研究对象选取于2017年6月至2018年12月在哈尔滨市第二医院接受诊断治疗的慢性肾脏病3d期-5期(CKD3d-5期)合并重症感染患者24例作为研究对象,将其依据随机数字表法的分组原则分为例数相等的两组(对照组、观察组),两组患者例数各为12例,对照组采用常规透析和维持性低通血液透析,观察组则应用常规抗炎治疗同时联合持续应用高通血液透析治疗,对比评价两组患者治疗前后血白细胞计数、中性粒细胞比、超敏C反应、C反应蛋白变化情况。结果治疗前,和对照组相比较,观察组患者血白细胞计数、中性粒细胞比、超敏C反应、C反应蛋白等指标并无明显差异(P>0.05);采取治疗措施以后,观察组患者治疗前后血白细胞计数、中性粒细胞比、超敏C反应、C反应蛋白等指标变化情况具有明显优势,组间数据对比差异显著(P<0.05)。结论在慢性肾衰合并重症感染患者的治疗过程中应用高通血液透析,对有效清除炎性介质具有积极意义,值得推广应用。
Objective To analyze and evaluate the clearance of inflammatory mediators by Qualcomm hemodialysis in patients with chronic renal failure and severe infection.Methods Twenty-four patients with CKD stage 3d+severely infected patients who were diagnosed and treated in our hospital from June to February 2018,2017 were selected as subjects.They were divided into groups according to the principle of random number table method.The two groups were equal in number(control group,observation group),and the number of patients in each group was 12,the control group was treated with conventional dialysis and maintenance low-pass hemodialysis,and the observation group was treated with conventional anti-inflammatory therapy and continuous application of Qualcomm.Hemodialysis treatment was used to compare the changes of white blood cell count,neutrophil ratio,hypersensitivity C reaction and C-reactive protein in the two groups before and after treatment.Results Before treatment,compared with the control group,there were no significant differences in white blood cell count,neutrophil ratio,hypersensitive C response,C-reactive protein and other indicators in the observation group(P>0.05).after treatment,observation group There were significant advantages in the changes of white blood cell count,neutrophil ratio,high-sensitivity C-reaction and C-reactive protein before and after treatment.There was significant difference in data between groups(P<0.05).Conclusion Patients with chronic renal failure and severe infection The application of Qualcomm hemodialysis in the treatment process has positive significance for the effective removal of inflammatory mediators and is worthy of popularization and application.
