摘要
目的探讨妊娠合并系统性红斑狼疮(SLE)的不良结局及其高危因素分析。方法选取2014年1月至2018年12月空军军医大学附属第一医院临床诊治的62例妊娠伴系统性红斑狼疮患者临床资料作为研究对象。依据是否妊娠失败分为两组,再依据妊娠成功不良妊娠结局进行分组,使用Logistic回归模型分析影响不良妊娠结局的危险因素。结果导致SLE伴妊娠出现妊娠失败的因素包括狼疮性肾炎、抗aCL阳性、SLEDAI、孕期SLE病情活动以及糖皮质激素使用等(P<0.05);影响分娩成功不良结局的危险因素包括雷诺现象、低补体C3及C4、抗Ro/SSA抗体阳性、抗La/SSB抗体阳性、狼疮性肾炎、SLEDAI评分、孕期SLE病情活动,数据差异具有统计学意义(P<0.05)。结论导致SLE合并妊娠患者不良妊娠结局的相关影响因素很多,针对该类患者需产科、风湿病专家会诊后制定处理方案,加强孕期、产后相关免疫指标的监测,提升和改善妊娠结局。
Objective To investigate the adverse outcomes and risk factors of pregnant patients complicated with systemic lupus erythematosus(SLE).Methods The clinical data of 62 patients with SLE in our hospital from January 2014 to December 2018 were analyzed retrospectively,which were divided into two groups according to whether or not the pregnancy failed,and then the patients with success pregnancies were grouped according to adverse pregnancy outcomes.Logistic regression model was used to analyze the risk factors for adverse pregnancy outcomes.Results The factors leading to pregnancy failure in SLE patients included lupus nephritis,anti-aCL-positive,SLEDAI,SLE activity during pregnancy,and glucocorticoid use(P<0.05).Risk factors adverse pregnancy outcomes included Raynaud′s phenomenon,low complement C3 and C4,anti-Ro/SSA antibody positive,anti-La/SSB antibody positive,lupus nephritis,SLEDAI score and SLE disease activity during pregnancy,with statistically significant differences(P<0.05).Conclusions There are many factors influencing the adverse pregnancy outcomes of SLE patients.Targeted treatment plan should be developed with the consultation of obstetricians and rheumatologists to strengthen the monitoring of pregnancy and postpartum related immune indicators in order to improve pregnancy outcomes.
