摘要
目的 探讨自我管理支持结合运动干预对肥胖型2型糖尿病患者健康行为及代谢指标的改善作用。方法 从该院2016年7月-2018年5月收治的肥胖型2型糖尿病患者中方便选取102例为研究对象,随机数字表法分为对照组与观察组,各51例,对照组接受常规护理,观察组在对照组基础上实施自我管理支持和运动干预,对两组患者干预前后健康行为及代谢指标进行观察。结果 两组干预前的膳食调配、血糖监测、合理运动、低血糖预防等健康行为评分差异无统计学意义(P>0.05),观察组干预后上述健康行为评分分别为(4.36±0.28)分、(4.39±0.25)分、(4.18±0.52)分、(4.51±0.48)分,明显较对照组(3.17±0.32)分、(2.97±0.36)分、(2.86±0.49)分、(2.73±0.57)分高(t=19.986、23.137、13.193、17.058,P=0.000);观察组干预后的糖脂代谢指标分别为(6.12±0.64)mmol/L、(7.75±0.38)mmol/L、(6.15±0.52)%、(1.54±0.29)mmol/L、(4.21±0.73)mmol/L、(2.32±0.41)mmol/L、(1.38±0.22)mmol/L,均优于对照组(7.45±0.68)mmol/L、(10.36±0.72)mmol/L、(6.97±0.43)%、(1.92±0.38)mmol/L、(5.17±0.53)mmol/L、(2.96±0.48)mmol/L、(1.14±0.28)mmol/L(t=10.171、22.894、8.678、5.677、7.599、7.240、4.813,P=0.000);观察组依从性为98.04%,明显较对照组84.31%高(χ^2=5.971,P=0.015)。结论 自我管理支持结合运动干预能提高肥胖型2型糖尿病患者健康行为,也能调节糖脂代谢指标,提高患者依从性,值得推广。
Objective To explore the effect of self-management support combined with exercise intervention on the improvement of health behavior and metabolic index in obese type 2 diabetic patients.Methods 102 patients with obesity type 2 diabetes admitted to the hospital from July 2016 to May 2018 were conveniently selected in the study.The random number table method was divided into in the control group and the observation group,with 51 cases in eacn group.The control group received routine care.The observation group implemented self-management support and exercise intervention on the basis of the control group,and observed the health behavior and metabolic indicators of the two groups before and after intervention.Results There were no statistically significant differences in the scores of dietary behaviors such as dietary allocation,blood glucose monitoring,reasonable exercise,and hypoglycemia prevention before intervention(P>0.05).The health behavior scores of the observation group were(4.36±0.28)points and(4.39±0.25)points,(4.18±0.52)points,(4.51±0.48)points,significantly higher than the control group(3.17±0.32)points,(2.97±0.36)points,(2.86±0.49)points,(2.73±0.57)points(t=19.986,23.137,13.193,17.058,P=0.000);the glucose and lipid metabolism indexes of the observation group were(6.12±0.64)mmol/L,(7.75±0.38)mmol/L,(6.15±0.52)%,(1.54±0.29)mmol/L,(4.21±0.73)mmol/L,(2.32±0.41)mmol/L,(1.38±0.22)mmol/L,better than the control group(7.45±0.68)mmol/L,(10.36±0.72)mmol/L,(6.97±0.43)%,(1.92±0.38)mmol/L,(5.17±0.53)mmol/L,(2.96±0.48)mmol/L,(1.14±0.28)mmol/L(t=10.171,22.894,8.678,5.677,7.599,7.240,4.813,P=0.000);observation group compliance was 98.04%,significantly higher than the control group 84.31%(χ^2=5.971,P=0.015).Conclusion Self-management support combined with exercise intervention can improve the healthy behavior of obese type 2 diabetic patients,also regulate glucose and lipid metabolism indicators,improve patient compliance,and is worth promoting.
