摘要
目的探讨无创血压监测系统在急诊科的应用。方法选取2018年9月至12月南方科技大学医院急诊科收治的38例创伤患者为研究对象,同时予以心电监护仪间歇式动脉血压监测(对照组)和无创血压监测系统监测(观察组)。比较两组监测数据。结果两组SAP、DAP、MAP的偏倚及标准差均符合美国国家标准学会和美国医疗器械促进协会推荐标准。SAP、DAP、MAP散点图显示,两种监测方式一致性良好。Pearson相关性分析:SAP、DAP、MAP的相关系数(r=0.92,0.87,0.88,P<0.05),显示两种监测方式相关性良好。观察组监测到19例出现低血压,低血压检出率50.0%(19/38);对照组监测到8例出现低血压,低血压检出率21.05(8/38)。观察组低血压检出率明显高于对照组,(χ^2=6.95,P=0.01)差异具有统计学意义。结论对急诊科患者予以CNAP连续无创血压监测和予以心电监护仪行间歇式血压监测的结果一致,但是CNAP能实时发现低血压的发作,避免漏诊或延误。
Objective To explore the application of non-invasive blood pressure monitoring system in emergency department.Methods A total of 38 trauma patients admitted to the Emergency Department of our hospital from September 2018 to December were selected as subjects.At the same time,ECG monitors were monitored by intermittent arterial blood pressure monitoring(control group)and non-invasive blood pressure monitoring system(observation group).The two sets of monitoring data was compared.Results The bias and standard deviation of SAP,DAP and MAP in both groups were in accordance with the recommended standards of the American National Standards Institute and the American Medical Device Promotion Association.SAP,DAP and MAP scatter plots show that the two monitoring methods have good consistency.Pearson correlation analysis showed that the correlation coefficients of SAP,DAP and MAP were 0.92,0.87 and 0.88(P<0.05),which showed that the two monitoring methods had good correlation.The observation group monitored 19 cases of hypotension,the detection rate of hypotension was 50.00%(19/38);the control group monitored 8 cases of hypotension,the detection rate of hypotension was 21.1(8/38).The detection rate of hypotension in the observation group was significantly higher than that in the control group(χ^2=6.95,P=0.01).Conclusion The results of CNAP continuous non-invasive blood pressure monitoring and intermittent blood pressure monitoring by ECG monitors are consistent in emergency department patients,but CNAP can detect the onset of hypotension in real time,avoiding missed diagnosis or delay,and it is worthy of promotion and application in emergency department.
