摘要
目的探讨三维适形放疗同步TP并联合细胞免疫治疗对Ⅲ期非小细胞肺癌的临床治疗效果。方法选取我院收治的60例Ⅲ期非小细胞肺癌患者(2015年1月~2016年6月)进行前瞻性研究,随机分为两组,各30例,对照组实施三维适形放疗+TP方案化疗,观察组在对照组基础上施加细胞免疫治疗,比较两组客观缓解率、细胞免疫功能指标、中远期存活率、局部复发率、不良反应发生率。结果(1)治疗后,观察组的客观缓解率、细胞免疫功能各项指标较对照组更高,差异有统计学意义(P<0.05)。(2)随访1、2年时,观察组的存活率较对照组更高,其局部复发率较对照组更低,差异有统计学意义(P<0.05)。(3)两组白细胞减少、恶心呕吐、放射性肺炎、血小板减少等不良反应发生率差异无统计学意义(P>0.05)。结论三维适形放疗+TP方案化疗+细胞免疫治疗可有效提高Ⅲ期非小细胞肺癌的近期疗效,提高其中远期存活率,还具有良好的安全性。
Objective To study and investigate the clinical effect of three-dimensional conformal radiotherapy andTP combined with cellular immunotherapy on stage III non-small cell lung cancer.Methods 60 patients with stage III nonsmall cell lung cancer(January 2015 to June 2016)who were admitted and treated in our hospital were selected for a prospective study.They were randomly divided into two groups,with 30 cases in each group.The control group was given three-dimensional conformal radiotherapy combined with TP regimen chemotherapy while the observation group was given cellular immune therapy on the basis of the control group.The objective remission rate,cellular immune function index,medium and long-term survival rate,local recurrence rate and incidence of adverse reactions were compared.Results(1)After treatment,the objective remission rate and cellular immune function of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).(2)After 1 or 2 years of follow-up,the survival rate of the observation group was higher than that of the control group,and the local recurrence rate of the observation group was lower than that of the control group.The difference was statistically significant(P<0.05).(3)The incidence of adverse reactions such as leucopenia,nausea and vomiting,radiation pneumonia and thrombocytopenia was not significantly different between groups(P>0.05).Conclusion Three-dimensional conformal radiotherapy and TP regimen chemotherapy combined with cellular immunotherapy can effectively improve the short-term curative effects and long-term survival rate of stage III non-small cell lung cancer.It also has good safety.
