摘要
目的探讨吴茱萸热奄包药熨腹部治疗ICU机械通气并发腹胀患者的临床效果。方法选取2017年10月至2018年10月在我院ICU接受机械通气治疗并发腹胀的患者80例为研究对象,采用随机数字表法分为观察组及对照组,每组40例。对照组患者给予常规处置;观察组患者在对照组治疗的基础上加用吴茱萸热奄包药熨脐周,疗程为7 d。比较两组患者治疗前后的腹压、APACHEⅡ评分、潮气量、肺顺应性、气道平台压;比较两组患者的腹胀治疗效果。结果治疗7 d后,两组患者的腹压、APACHEⅡ评分均显著降低,观察组患者的腹压、APACHEⅡ评分均显著低于对照组,差异有统计学意义(P<0.05);观察组患者的潮气量、肺顺应性显著高于对照组,气道平台压显著低于对照组,差异有统计学意义(P<0.05)。观察组患者的腹胀治疗总有效率(87.5%)显著高于对照组(62.5%),临床疗效优于对照组,差异有统计学意义(P<0.05)。结论采用吴茱萸热奄包对机械通气治疗并发腹胀患者进行药熨脐周,能明显缓解腹胀,改善胃肠功能及肺功能,临床疗效显著,值得推广应用。
Objective To explore the efficacy of Fructus Evodiae hot pack compressed on abdomen treating ICU mechanical ventilation complicated with abdominal distension.Methods Eighty patients received mechanical ventilation complicated with abdominal distension in ICU in our hospital from October 2017 to October 2018 were selected as the objects.They were divided into observation group and control group by random number table method,with 40 cases in each group.The control group were given routine treatment,whereas the observation group were treated with Fructus Evodiae hot pack compressed around the navel on the basis of the treatment of the control group,with seven-day course of treatment.The abdominal pressure,APACHE Ⅱ score,tidal volume,lung compliance,and airway platform pressure were compared between the two groups before and after treatment,and the efficacy of abdominal distension between the two groups was compared.Results After 7 days of treatment,the abdominal pressure and APACHE Ⅱ scores of the two groups were significantly decreased.The abdominal pressure and APACHE Ⅱ scores in the observation group were significantly lower than those in the control group.The difference was statistically significant(P<0.05).The tidal volume and lung compliance in the observation group were significantly higher than those in the control group,whereas the airway platform pressure was significantly lower than that of the control group,with statistically significant difference(P<0.05).The total effective rate of abdominal distension treatment in the observation group(87.5%)was significantly higher than that in the control group(62.5%),and the efficacy was better than that of the control group,with statistically significant difference(P<0.05).Conclusion Fructus Evodiae hot pack compressed around the navel treating mechanical ventilation complicated with abdominal distension can significantly relieve the condition of abdominal distension,improve gastrointestinal function and lung function,and efficacy is significance.It is worth
