摘要
目的通过对韶关市某医院ICU患者进行三管监测,分析引发其相关感染的病原菌及预防方式。方法对2014年1月至2018年12月入住lCU的患者3009例进行三管相关感染监测,包括使用感染例次率、感染部位统计、造成感染病原菌统计及三管相关感染统计。结果在3009例患者中,发生三管相关感染的196例,平均感染例次率为6.51%。呼吸道感染例次数最多,占总例次数的44.90%;其次为泌尿道感染,占26.53%;再次为血管相关感染,占19.39%;另有其他部位感染18例,占9.18%。肺炎克雷伯菌、白假丝酵母菌及鲍曼不动杆菌造成的感染共101例,感染率为51.54%。5年内导尿管相关泌尿系感染(CAUTI)平均发生率为2.25‰、导管相关血流感染(CRBSI)平均发生率为1.34‰、呼吸机相关性肺炎(VAP)平均发生率为5.17‰。结论肺炎克雷伯菌为ICU三管相关感染的主要因素,有效控制病原菌可预防三管相关感染,降低感染率。
Objective To analyze the pathogens and prevention methods of the related infections by conducting 3-tube monitoring on ICU patients in a hospital in Shaoguan city.Methods 3-month-related infection surveillance was performed in 3009patients who admitted to lCU from January 2014to December 2018,including the incidence of infection,the statistics of infection sites,infectious pathogens and 3-tube related infections.Results Among the 3009patients,196had 3-tube-associated infections,with an average infection rate of 6.51%.Respiratory tract infection had the largest number of cases,accounting for 44.90%,followed by urinary tract infection(26.53%),vascular-related infection(19.39%),and other parts(18cases,9.18%).There were 101cases of infection caused by klebsiella pneumoniae,candida albicans and acinetobacter baumannii,the infection rate was 51.54%.The average incidence of catheter-related urinary tract infection(CAUTI)within 5years,catheter-related bloodstream infection(CRBSI)and ventilator-associated pneumonia(VAP)was 2.25‰,1.34‰and 5.17‰respectively.Conclusion Klebsiella pneumoniae is the main cause of 3-tube related infections in ICU,the 3-tube related infections can be prevented by controlling the pathogens effectively in order to reduce the infection rate.
