摘要 目的探讨血栓弹力图(thrombelastogram, TEG)在大量输血中的应用价值。方法选择2014年7月至2018年6月于玉林市红十字会医院行外科手术的患者84例,随机分为TEG组(n=42)和常规组(n=42),另选取同期门诊体检的健康人群作为对照组(n=42)。比较3组患者手术失血量和成分输血用量、输血评估指标和TEG参数、术后并发症发生情况,分析输血评估指标和TEG参数相关性。结果 TEG组所用PLT、血浆及冷沉淀的量少于常规组,术后并发症发生率低于常规组,差异均有统计学意义(P<0.05);PT、APTT与TEG的R呈正相关;FIB与TEG的MA呈正相关,与K呈负相关;PLT与TEG的α-Angle、MA呈正相关,差异均有统计学意义(P<0.05)。结论应用TEG指导成分输血具有鉴定价值,值得推广应用。 Objective To investigate the clinical value of thrombelastogram(TEG) for massive blood transfusion.Methods A total of 84 patients who underwent surgery in Yulin Red Cross Hospital from July 2014 to June 2018 were randomly divided into TEG group(n=42) and routine group(n=42). Meanwhile, healthy people who underwent physical examination during the same period were selected as the control group(n=42). The operative blood loss and component transfusion dosage, transfusion evaluation indexes, TEG parameters and postoperative complications were compared among groups. The correlation between blood transfusion evaluation indexes and TEG parameters was analyzed. Results The amount of PLT,plasma and cryoprecipitation used in the TEG group were lower than those in the conventional group, and the incidence of postoperative complications was lower than that in the conventional group, with statistically significant differences(P < 0.05).PT and APTT were positively correlated with R of TEG. FIB was positively correlated with MA of TEG and negatively correlated with K. PLT was positively correlated with alpha-angle and MA of TEG. All the differences were statistically significant(P < 0.05). Conclusions The application of TEG in blood transfusion has the value of identification and is worth popularizing.
