动态心电图监测传导阻滞与窦房结功能不良患者起搏器植入术后的价值观察 被引量:4

摘要 目的 探究动态心电图监测传导阻滞与窦房结功能不良患者起搏器植入术后的应用价值.方法 100例行DDD型起搏器植入的患者作为研究对象,将存在传导阻滞的患者作为传导阻滞组(50例),存在窦房结功能不良的患者作为窦房结功能不良组(50例).两组患者均采用动态心电图监测.比较两组患者的自身心律失常及心室安全起搏的检出率.结果 窦房结功能不良组患者的频发房性早搏、房性心动过速的检出率分别为30.00%(15/50)、34.00%(17/50),均高于传导阻滞组患者的4.00%(2/50)、6.00%(3/50),差异具有统计学意义(P<0.05).窦房结功能不良组患者的室性早搏、阵发性房颤、成对室性早搏、短阵室速、多源性室性早搏检出率分别为22.00%(11/50)、10.00%(5/50)、16.00%(8/50)、10.00%(5/50)、10.00%(5/50),与传导阻滞组患者的24.00%(12/50)、8.00%(4/50)、14.00%(7/50)、8.00%(4/50)、10.00%(5/50)比较,差异均无统计学意义(P>0.05).窦房结功能不良组患者心室安全起搏的检出率为26.00%(13/50),高于传导阻滞组患者的10.00%(5/50),差异具有统计学意义(P<0.05).及时调整心房感知器的灵敏度后心室安全起搏的检出率显著降低.结论 窦房结功能不良和传导阻滞患者治疗过程中植入起搏器,再充分借助动态心电图来监测自身情况,可以让患者对自身心律失常有更为准确的了解,对于心律失常症状的有效防治及保障患者的生命安全具有重要意义,适宜推广使用. Objective To investigate the application value of dynamic electrocardiogram monitoring in patients with conduction block and sinus node dysfunction after pacemaker implantation.Methods 100 patients with DDD pacemaker implantation were taken as study subjects.Patients with conduction block were treated as conduction block group(50 cases)and patients with sinus node dysfunction were treated as sinus node dysfunction group(50 cases).Both groups received dynamic electrocardiogram monitoring.The detection rates of arrhythmia and ventricular safe pacing were compared between the two groups.Results The detection rates of frequent atrial premature beats and atrial tachycardia were 30.00%(15/50)and 34.00%(17/50)respectively in sinus node dysfunction group,which was higher than 4.00%(2/50)and 6.00%(3/50)in conduction block group,and their difference was statistically significant(P<0.05).The detection rate of premature ventricular beats,paroxysmal atrial fibrillation,paired premature ventricular beats,short paroxysmal ventricular tachycardia and multiple premature ventricular beats was 22.00%(11/50),10.00%(5/50),16.00%(8/50),10.00%(5/50)and 10.00%(5/50)respectively in sinus node dysfunction group,which was 24.00%(12/50),8.00%(4/50),14.00%(7/50),8.00%(4/50)and 10.00%(5/50)in conduction block group.Their difference was not statistically significant(P>0.05).The detection rate of ventricular safe pacing was 26.00%(13/50)in sinus node dysfunction group,which was higher than 10.00%(5/50)in conduction block group,and the difference was statistically significant(P<0.05)The detection rate of ventricular safe pacing decreased significantly after adjusting the sensitivity of atrial perceptron in time.Conclusion For the treatment of patients with sinus node dysfunction and conduction block,pacemaker implantation and dynamic electrocardiogram(DCG)monitoring of their own conditions can enable patients to have a more accurate understanding of arrhythmias.It is of great significance for the effective prevention and treatment of arrhythmia
