血清HE4、CA125联合检测对卵巢上皮性癌的诊断价值 被引量:2

Diagnostic Value of Combined Detection of Serum HE4 and CA125 in Epithelial Ovarian Cancer

摘要 目的探讨血清HE4、CA125单独或联合检测对卵巢癌的诊断价值。方法采用放射免疫法及ELISA法分别检测卵巢癌患者(卵巢癌组)与卵巢良性肿瘤患者(良性肿瘤组)血清中HE4和CA125水平,并计算其敏感性、特异性,ROC曲线分析其诊断价值。结果卵巢癌组HE4、CA125水平明显高于良性肿瘤组(P均<0.05)。对卵巢癌的诊断,CA125检测敏感性为75.93%,特异性为61.64%;HE4检测敏感性为90.74%,特异性为95.21%;二者联合检测敏感性为92.60%,特异性为59.59%。HE4的特异性高于CA125及二者联合(P均<0.01)。HE4检测、CA125检测、HE4与CA125联合检测的ROC曲线下面积分别为0.961、0.712、0.959。结论HE4检测或联合CA125检测与单独CA125检测相比可以进一步提高卵巢癌诊断水平,适用于卵巢癌和良性肿瘤的鉴别诊断。 Objective To investigate the diagnostic value of serum HE4,CA125 alone or combination in the diagnosis for ovarian cancer.Method The level of serum HE4 and CA125 in patients with ovarian cancer(the ovarian cancer group)and patients with benign ovarian tumor(benign tumor group)were detected by radioimmunoassay and ELISA method,respectively.The sensitivity,specificity were calculated,and ROC curve was used to analyze its diagnostic value.Results The level of HE4 and CA125 in the ovarian cancer group were apparently higher than that of benign tumor group(both P<0.05).As for the diagnosis for the ovarian cancer,the sensitivity of CA125 inspection was 75.93%,the specificity was 61.64%;The sensitivity of HE4 inspection was 90.74%,the specificity was 95.21%;while the sensitivity of the combination was 92.60%,the specificity was 59.59%.The specificity of HE4 was higher than that of the CA125 alone and its combination with HE4(P<0.01).The ROC area under the curve(AUC)of HE4 inspection,CA125 inspection and the combination inspection between HE4 and CA125 was 0.961,0.712 and 0.959,respectively.Conclusion HE4 alone or combination with CA125 can further improve the diagnostic level of ovarian cancer compared with the CA125 alone,hence it is suitable for the differential diagnosis of ovarian cancer and benign tumor.
