摘要
目的对我院近年来孕妇感染HBV状况及临床特征加以调查研究,以指导乙肝母婴阻断工作。方法纳入2016年9月1日至2018年8月30日在西安医学院第二附属医院就诊的门诊及住院孕产妇共4908例,对其HBsAg进行检查,对于阳性患者进一步进行血清学标志物、病毒载量和肝功能的检查,并进行统计学分析。结果检出HBsAg阳性者204例,阳性率为4.16%,HBsAg阳性率在年龄较大组更高。HBsAg阳性患者中,HBeAg阳性者占比25.5%,DNA高病毒载量(≥106 IU/mL)者占比19.6%。HBeAg阳性和阴性两组间病毒DNA载量水平差异有统计学意义(P<0.001,χ^2=35.9),而ALT、AST、GGT和TBiL结果的正常组与异常组两组间的DNA载量水平差异均无统计学意义(P>0.05)。结论育龄妇女在孕前应接受乙肝表面标志物检查,对于表面抗原阳性妇女需要进一步监测DNA病毒载量和肝功指标,保证孕前、孕期病毒复制处于较低水平,提高新生儿阻断成功率。
Objective To investigate the HBV infection and its clinical characteristics among pregnant and delivery women in our hospital,so to guide the procedure of mother-to-child blockade of hepatitis B.Methods 4908 pregnant and delivery women in Second Affiliated Hospital of Xi'an Medical College were included in the study from September,2016 to August,2018.Serum HBsAg detection was conducted for each subject.Other serological markers,viral load and liver function were further examined among HBsAg positive patients,and statistical analysis were performed.Results The positive rate of HBsAg in 4,908 women was 4.16%.The positive rate of HBsAg was higher in order group.Among HBsAg positive patients,25.5%of them were HBeAg positive,and 19.6%of them were high-DNA viral load(>106 IU/mL).There was significant difference in viral DNA load between the HBeAg positive and negative groups(P<0.001,χ2=35.9),while there was no significant difference in ALT,AST,GGT and TBiL between the normal and abnormal groups(P>0.05).Conclusion Women should be examined for hepatitis B surface markers before pregnancy.For HBsAg positive women,antigen,DNA viral load and liver function index should be further monitored to ensure that viral replication is at a low level before and during pregnancy,so to improve the success rate of neonatal blockade.
