摘要
目的分析人乳头瘤病毒DNA(HPV-DNA)联合鳞状细胞癌抗原(SCC-Ag)、癌胚抗原(CEA)、人附睾蛋白4(HE4)测定在宫颈癌患者诊断中的价值研究。方法选取宫颈癌患者(宫颈癌组)、宫颈癌前病变患者(宫颈癌前病变组)、宫颈炎患者(宫颈炎组)各60例,并取同期健康体检女性100例纳入对照组,观察宫颈癌不同分期与4组SCC-Ag、CEA、HE4、HPV-DNA及联合检测阳性率,比较4组血清SCC-Ag、CEA、HE4表达水平、SCC-Ag、CEA、HE4、HPV-DNA单独检测与联合检测灵敏度、特异度、准确度。结果宫颈癌前病变组与宫颈癌组血清SCC-Ag、CEA、HE4表达水平明显高于对照组与宫颈炎组(P<0.05),且宫颈癌组明显高于宫颈癌前病变组(P<0.05);宫颈癌前病变组与宫颈癌组SCCAg、CEA、HE4、HPV-DNA及联合检测阳性率均明显高于对照组、宫颈炎组(P<0.05),且宫颈癌组明显高于宫颈癌前病变组(P<0.05);Ⅰ/Ⅱ期患者SCC-Ag、CEA、HE4、HPV-DNA检测阳性率明显高于Ⅲ/Ⅳ期患者(P<0.05);联合检测灵敏度、准确度明显高于SCC-Ag、CEA、HE4、HPV-DNA单独检测(P<0.05)。结论随着宫颈癌的发展,阳性检出率明显增高,HPV-DNA联合SCC-Ag、CEA、HE4检测能够提高宫颈癌临床诊断敏感度与准确度,对有效诊治宫颈病变具有重要意义。
Objective To analyze the value of human papilloma virus DNA(HPV-DNA)combined with squamous cell carcinoma antigen(SCC-Ag),carcinoembryonic antigen(CEA)and human epididymis protein 4(HE4)in the diagnosis of cervical cancer.Methods 60 patients with cervical cancer(cervical cancer group),60 patients with precancerous cervical lesions(precancerous cervical lesion group)and 60 patients with cervicitis(cervicitis group)were selected.100 healthy women were included in the control group.The positive rates of SCC-Ag,CEA,HE4,HPV-DNA and combined detection for different stages of cervical cancer and 4 groups were observed.The expression levels serum SCC-Ag,CEA and HE4,sensitivities,specificities and accuracy rates of single and combined detection of SCC-Ag,CEA,HE4 and HPV-DNA were compared among the 4 groups.Results The expression levels of serum SCC-Ag,CEA and HE4,positive rates of SCC-Ag,CEA,HE4,HPV-DNA and combined detection were significantly higher in precancerous cervical lesion group and cervical cancer group than in the control group and cervicitis group(P<0.05),which was significantly higher in cervical cancer group than in the precancerous cervical lesion group(P<0.05).The positive rates of SCC-Ag,CEA,HE4 and HPV-DNA detection in patients at stageⅠ/Ⅱwere significantly higher than those in patients at stageⅢ/Ⅳ.The sensitivity and accuracy of combined detection were significantly higher than those of SCC-Ag,CEA,HE4 or HPV-DNA detection alone.Conclusion With the development of cervical cancer,the positive detection rate increases significantly.HPV-DNA combined with SCC-Ag,CEA and HE4 detection can improve the clinical diagnostic sensitivity and accuracy of cervical cancer,which is of great significance for effective diagnosis and treatment of cervical lesions.
