摘要
目的探讨多层螺旋CT对足部细微及隐匿性骨折患者的诊断价值。方法回顾性分析2016年3月-2018年7月期间在该院接受治疗的106例足部细微及隐匿性骨折患者的临床资料,所有入选者均已接受多层螺旋CT检查以及DR检查,记录诊断结果以及骨折部位。结果多层螺旋CT对足部细微及隐匿性骨折的确诊率高于DR平片检查结果,漏诊率与可疑率均低于DR平片检查结果,差异有统计学意义(χ2=18.559、12.500、4.802,P=0.000、0.000、0.028);DR平片检查无法对患者骨折部位给予明确判断,且无法对多处骨折给予详细报告;经多层螺旋CT检查显示,73例确诊患者中骨折94处:其中跖骨占43.62%,楔骨占17.02%,距骨占12.77%,舟骨占10.64%,骰骨占9.57%,跟骨占6.38%。结论多层螺旋CT为临床诊断足部细微及隐匿性骨折患者的有效手段,具有较高的诊断准确率,且可明确骨折部位,利于为临床治疗提供相关依据。
Objective To explore the diagnostic value of multislice spiral CT in patients with subtle and occult foot fractures. Methods Clinical data of 106 patients with subtle and occult foot fractures treated in our hospital from March2016 to July 2018 were retrospectively analyzed. Results The rate of diagnosis of subtle and occultable foot fractures by multislature spiral CT was higher than that by DR radiographs, and the rate of missed diagnosis and suspicious rate were lower than those by DR radiographs. The difference was statistically significant(χ2=18.559, 12.500, 4.802, P =0.000,0.000, 0.028). DR plain film examination could not give a clear judgment on the fracture site of patients, and could not give a detailed report on multiple fractures;Multi-slice spiral CT examination showed 94 fractures in 73 patients:metatarsal bone accounted for 43.62%, wedge bone 17.02%, talus bone 12.77%, scaphoid bone 10.64%, cuboid bone9.57%, calcaneus bone 6.38%. Conclusion Multislice spiral CT is an effective method for the clinical diagnosis of patients with subtle and occult foot fractures.
