摘要
目的探讨肺结核患者外周血和支气管肺泡灌洗液(BALF)中Vδ2^+T淋巴细胞比例的临床意义。方法采集37例活动性肺结核患者(活动性肺结核组)、45例潜伏性肺结核患者(潜伏性肺结核组)及40例健康体检者的静脉血标本,同时采集活动性肺结核组和潜伏性肺结核组的BALF标本。检测研究对象外周血及BALF(除外健康体检者)中Vδ2^+T淋巴细胞占总淋巴细胞百分比,以及血清γ干扰素(IFN-γ)、白细胞介素(IL)4、IL-10水平。结果潜伏性肺结核组、活动性肺结核组患者外周血Vδ2^+T淋巴细胞占总淋巴细胞百分比低于健康组;活动性肺结核组患者BALF中Vδ2^+T淋巴细胞占总淋巴细胞百分比均低于潜伏性肺结核组(均P<0.05)。潜伏性肺结核组和活动性肺结核组患者血清IFN-γ、IL-4、IL-10水平均高于健康组,且活动性肺结核组患者血清IL-4、IL-10水平均高于潜伏性肺结核组(均P<0.05)。结论肺结核患者外周血Vδ2^+T淋巴细胞比例下降,其可能通过调节血清IL-4、IL-10的水平,参与肺结核的发生发展进程。
Objective To explore the clinical significance of Vδ2^+T lymphocytes percentage in peripheral blood and bronchoalveolar lavage fluid(BALF)of patients with pulmonary tuberculosis.Methods Venous blood samples were collected from 37 patients with active pulmonary tuberculosis(active pulmonary tuberculosis group),45 patients with latent pulmonary tuberculosis(latent pulmonary tuberculosis group)and 40 healthy check-up persons(healthy group),and concurrently BALF samples of the active pulmonary tuberculosis group and the latent pulmonary tuberculosis group were collected.The ratio of Vδ2^+T lymphocytes to total lymphocytes in peripheral blood and BALF(except for the healthy subjects),as well as serum levels ofγinterferon(IFN-γ),interleukin(IL)4 and IL-10 were detected in the subjects.Results The latent pulmonary tuberculosis group or the active pulmonary tuberculosis group had a lower ratio of Vδ2^+T lymphocytes to total lymphocytes in peripheral blood than the healthy group;the active pulmonary tuberculosis group reported a lower ratio of Vδ2^+T lymphocytes to total lymphocytes in BALF than the latent pulmonary tuberculosis group(all P<0.05).The levels of serum IFN-γ,IL-4 and IL-10 in the latent and active pulmonary tuberculosis groups were higher than those in the healthy group,and the levels of serum IL-4 and IL-10 in the active pulmonary tuberculosis group were higher than those in the latent pulmonary tuberculosis group(all P<0.05).Conclusion The proportion of Vδ2^+T lymphocytes in peripheral blood exhibits a reduction in patients with pulmonary tuberculosis,and it may participate in the occurrence and development of pulmonary tuberculosis by regulating the levels of serum IL-4 and IL-10.
