摘要
目的:探讨利拉鲁肽治疗类风湿关节炎合并2型糖尿病的临床效果。方法:选取2013年2月至2016年12月河北省廊坊市人民医院收治的200例类风湿关节炎合并2型糖尿病患者,按随机数字表法将其分为观察组和对照组,每组100例。对照组给予甘精胰岛素联合甲氨蝶呤治疗,观察组在对照组治疗基础上加用利拉鲁肽治疗。比较两组治疗前、后空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、类风湿因子(RF)、C反应蛋白(CRP)水平及红细胞沉降率(ESR)、类风湿关节炎疾病活动相关指标、晨僵时间、关节疼痛数和肿胀关节数。结果:治疗后,两组28个关节疾病活动评分(DAS28)、简化疾病活动指数(SDAI)、临床疾病活动指数(CDAI)、ESR及血清FPG、2 h PG、HbA1c、TG、LDL-C、CRP、RF水平均较治疗前降低,HDL-C浓度升高,晨僵时间缩短,关节疼痛数和肿胀关节数减少,且观察组改善更为明显(均P<0.05)。结论:利拉鲁肽治疗类风湿关节炎合并2型糖尿病能有效调节患者的血糖和血脂水平,促进疾病活动度、临床症状及炎症反应的改善。
Objective:To investigate the clinical effect of liraglutide on rheumatoid arthritis complicated with type 2 diabetes.Methods:A total of 200 patients with rheumatoid arthritis and type 2 diabetes in our hospital from February 2013 to February 2016 were selected,and randomly divided into an observation group and a control group,with 100 cases in each group.The levels of fasting plasma glucose(FPG),postprandial 2 h blood glucose(2 h PG),glycohemoglobin(HbA1 c),tiglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),rheumatoid factor(RF)and C-reactive protein(CRP)as well as disease activity score-28(DAS28),simplified disease activity index(SDAI),clinical disease activity index(CDAI),duration of morning stiffness,number of joint pain or swollen joints and erythrocyte sedimentation rate(ESR)were compared.Results:After treatment,DAS28,SDAI,CDAI,ESR,morning stiffness time,the number of joint pain and swelling joints,and the serum levels of FPG,2 h PG,HbA1 c,TG,LDL-C,CRP and RF were decreased,while the serum HDL-C concentration was increased in both groups(P<0.05).These changes were more marked in the observation group(P<0.05).Conclusion:Liraglutide could effectively regulate blood glucose and blood lipid levels,and promote the improvement of disease activity,clinical symptoms and inflammatory response in patients with rheumatoid arthritis complicated with type 2 diabetes.
