摘要
目的:探讨高频超声在弥漫硬化型甲状腺乳头状癌诊断中的价值。方法:以手术病理为参照,回顾13例弥漫硬化型甲状腺乳头状癌的超声图像特征,分析其诊断价值。结果:13例患者平均年龄27岁,男女比例3:10。13例患者单侧叶者5例,双侧叶者8例。超声表现为甲状腺肿大(13/13,100%),患侧甲状腺腺叶均表现为无明显占位,呈实质回声不均、紊乱、弥漫性或散在沙粒样钙化,颈部中央区异常淋巴结(12/13,92.3%),颈侧区异常淋巴结(10/13,76.9%)。5例行超声弹性成像,评分均≥3分,1例行超声引导下颈侧区淋巴结穿刺明确诊断,其余12例超声均直接作出诊断。结论:年轻女性、甲状腺腺体肿大、质地硬、弥漫性沙粒样钙化、颈部异常淋巴结是弥漫硬化型甲状腺乳头状癌的特征性表现,必要时对异常淋巴结进行穿刺活检可以进一步明确诊断。
Objectives To discuss the high-frequency ultrasonic diagnosis value of papillary thyroid carcinoma with diffuse sclerosis.Methods By regarding surgery pathology as the reference,ultrasonic image features of 13 papillary thyroid carcinoma patients with diffuse sclerosis were reviewed to analyze their diagnostic value.Results The average age of 13 patients was 27 years old.The male/female ratio was 3:10.In 13 patients,5 patients had the unilateral lobe and 8 patients had the bilateral lobe.The ultrasonic findings included thyroid enlargement(13/13,100%).Thyroid gland at the affected side showed no obvious occupation and revealed uneven and disordered substantial echo,diffusivity or sand-like calcification.The central zone of the neck had abnormal lymph gland(12/13,92.3%)and lateral neck had abnormal lymph gland(10/13,76.9%).5 patients conducted ultrasonic elastic imaging.The score was more than 3 scores.1 patient conducted lymph gland puncture under the ultrasonic guidance to define diagnosis.Other 12 patients’ultrasound could make a direct diagnosis.Conclusions The characteristic performance of papillary thyroid carcinoma with diffuse sclerosis includes young women,swelling thyroid gland,hard texture,diffuse sand-like calcification and abnormal lymph gland on the neck.If necessary,needle biopsy of abnormal lymph gland can further clarify a diagnosis.
